      目的 明确踝关节MRI检查对踝关节创伤性病变的诊断价值。方法 回顾性分析经证实的78例踝关节MR图像,重点评价踝关节MRI对外侧副韧带、距骨软骨、肌腱以及滑膜病变的诊断能力。结果 ①MRI对距腓前韧带损伤具有非常高的诊断敏感性(91%),对跟腓韧带损伤的敏感性则约为50%~75%。②MRI对于距骨骨软骨损伤总的敏感性仅为67%,但对同时合并软骨下骨改变的软骨损伤非常敏感(100%)。③MRI诊断肌腱病变准确。④MRI诊断滑膜增生以及前外撞击综合征的能力有限。结论 MRI对于踝关节创伤性病变有一定的诊断价值,但同时存在很多诊断不足。
英文摘要:
      Objective To determine the value of MRI in diagnosing various diseases of the traumatized ankle. Methods MRI examinations of 78 cases with traumatic ankle disease, which were proved by subsequently arthroscopy, were retrospectively analyzed. The emphases were aimed to the lateral collateral ligament, cartilage of talus, tendons of ankle, and diseases of synovium. Results ①MRI had a very high sensitivity of 91% to the injuries of anterior talofibular ligament, but a relatively low sensitivity of 50%-75% to calcaneofibular ligament injuries. ②In a whole, sensitivity of MRI to talar cartilage injuries was only about 67%. However, it improved to 100% when only in those cases that subcartilage bone was also involved. ③MRI was accurate in revealing tendon's diseases. ④MRI was incompetent to display hyperplastic synovium and diagnose anterolateral impingement of ankle. Conclusion MRI was a valuable diagnostic method for many common traumatic ankle injuries. However, it should be kept in mind that MRI had also many insufficiencies simultaneously.
