目的 探讨微波消融脑组织的可行性及量效关系,为超声引导微波消融脑肿瘤的临床应用提供实验依据。方法 以不同微波功率及消融时间的组合随机作用于15只家猫脑组织,术后1 h内超声探查消融灶,术后48 h取脑组织观察病理改变、测量消融直径及脑水肿带宽。结果 除1只猫术后46 h死亡外其余均在16 h内苏醒;术后48 h病理呈特征性改变:由中心向外周依次是残腔、凝固性坏死、水肿区、水肿外带;消融直径(R)及脑水肿带宽(E)与输出功率(P)、消融时间(T)线性相关,得到回归方程R=0.287P+3.853×10-2T 及E=-0.901+5.140×10-2P+1.513×10-2T; 术后1 h内超声探查消融灶中心见小片状强回声,周边呈边界不清的低回声区,消融灶内无血流信号。结论 微波消融脑组织创伤小、安全可行,根据输出功率和消融时间可以调控微波消融范围,并预测脑水肿程度,20~30 W是合适的微波输出功率。 |

Objective To explore the feasibility and does-effect relationship of microwave ablation (MA) for brain tissue. Methods A total of 15 domestic cat brains underwent MA randomly at different output power and ablation duration. The lesions were detected with ultrasonography within 1 hour after MA. Cats were sacrificed 48 hours postoperatively. Morphology and microscope structure changes including ablation diameter and cerebral edema bandwidth were observed. Results All cats came around within 16 hours postoperatively except one died 46 hours after MA. From the center to peripheral area of the lesions, the pathology changes were characterized as leftover cavity, coagulative necrosis, cerebral edema band and extra-band of cerebral edema. Ablation diameter (R) and cerebral edema bandwidth (E) were linear correlation with microwave output power (P) and ablation duration (T). The regressive equation were R=0.287P+3.853×10-2T and E=-0.901+5.140×10-2P+1.513×10-2T, respectively. Ultrasonographic features of the lesions were presented as patchy hyperecho, surrounded with ill-defined hypoechoic area, without blood flow signal. Conclusion Microwave ablation for brain tissue is minimally invasive and feasible. The ablation diameter can be controlled based on output power and ablation duration, and the severity of cerebral edema can also be predicted. 20-30 W is suitable output power range. |
