      目的 探讨全脑DTI直方图分析对皮质下缺血性痴呆(SVD)患者的诊断价值。方法 对10例SVD患者和14例健康志愿者行常规MRI和扩散张量成像(DTI)检查,获得全脑平均扩散率(MD)和各向异性分数(FA)图像后,分别绘制出全脑MD和FA 直方图并对其进行分析。结果 与正常对照比较,SVD患者平均全脑MD直方图明显右移、峰高降低,其平均MD、MD直方图峰位置明显增高,而MD直方图峰高明显降低(全脑平均MD,P=0.023;峰高,P=0.001;峰位置,P=0.046)。在SVD患者,全脑平均MD(r=-0.671, P=0.034)、MD直方图峰位置(r=-0.878,P=0.001)与简易智能状态检查(MMSE)评分具有显著相关性。结论 SVD患者的弥散与正常对照组明显不同,MD直方图分析可作为评价疾病严重性的指标。

      Objective To investigate the value of whole brain diffusion tensor imaging (DTI) histogram in the diagnosis of subcortical vascular dementia (SVD). Methods Conventional MRI and DTI scans were performed in 10 patients with SVD and 14 normal volunteers.After obtaining the whole brain mean diffusivity (MD) and fractional anisotropy (FA) images, the whole brain MD and FA histograms were drawn and analyzed. Results The pattern of the whole brain average MD histograms in SVD patients were different from those of control subjects. Compared with those of control subjects, the average MD histogram in patients with SVD shifted to the right with a lower peak. The average MD of the whole brain and the MD histogram peak location in patients with SVD was significantly higher than that the control group (P=0.023, P=0.046). But the MD histogram peak height was significantly lower than the control group (P=0.001). There were correlated between the average MD (r=-0.671, P=0.034), and the MD histogram peak location (r=-0.878, P=0.001) in patients with SVD and the mini-mental state examination (MMSE) scores. Conclusion Patients with SVD have obviously brain abnormal diffusion, and some of the MD histogram-derived measures may be used to monitor the severity of this disease.
