      目的 探讨乳腺良恶性病灶动态增强MRI半定量参数与病灶微血管密度(MVD)的相关性。方法 44例乳腺病人经病理证实分为良恶性两组,经过MRI动态增强后计算时间-信号曲线半定量参数,并对所有病灶进行免疫组化CD34染色计数微血管密度进行评分,评价两者的相关性。结果 乳腺癌24例,乳腺良性病灶20例,恶性组的早期强化率(Ee)、最大强化率(Emax)、最大强化速率(Vmax)、最大相对强化斜率(Eslop)及最大排泄率(Ewash)与微血管密度有明显正相关性(r=0.632,0.783,0.651, 0.792,0.723,P<0.01),达峰时间(Tmax)与微血管密度呈明显的负相关(r=-0.785,P<0.01),而良性组中仅有早期强化率(Ee)、早期强化速率(Ve)、最大强化率(Emax)与微血管密度有明显正相关性(r=0.728,0.708,0.850,P<0.01)。结论 MR T1动态增强是无创检测肿瘤微血管改变有效诊断方式,其强化参数与微血管密度具有明显的相关性。

      Objective To evaluate the correlation between semi-quantitative parameters from time-intensity curve of dynamic contrast-enhanced MR imaging (DCE-MRI) and microvessel density (MVD) of the lesions. Methods Forty-four patients with breast lesions were classified into two groups according to the pathologic reports: the malignant group and benign group. All cases were underwent DCE-MRI scanning. The semi-quantitative parameters of DCE-MRI were calculated based on time-intensity curves. Surgical specimens were analyzed with immunohistochemistry to measure of the CD34 MVD using the anti-CD34 antibody clone. The correlation of semi-quantitative parameters and the expression levels of MVD were analyzed. Results There were twenty-four malignant breast lesions and twenty benign breast lesions. The semi-quantitative parameters of malignant group had significant positive correlation with MVD, that as followed: Ee, Emax, Vmax, Eslop and Ewash (r=0.632, 0.783, 0.651, 0.792, 0.723, P<0.01), Tmax had significant negative correlation (r=-0.785, P<0.01), though the semi-quantitative parameters of benign group, Ee, Ve and Emax had positive correlation with MVD (r=0.728, 0.708, 0.850, P<0.01). Conclusion DCE-MRI was an atraumatic diagnose method to detect the microvessel of breast lesions. The semi-quantitative parameters of time-intensity curve were correlated with MVD.
