      目的 对宫颈癌根治术前病例作矢状动态增强扫描,进行前瞻性分析,评估动态增强扫描对宫颈癌的诊断价值。方法 收集宫颈活检确诊为宫颈癌21例,行常规MR及动态增强扫描,描绘病灶及周围组织动态增强扫描的时间-信号强度曲线,评估动态增强对宫颈癌分期的价值。结果 对比剂注入16s后,20例病灶均有不均匀强化,16例(80%)强化病灶信号较子宫肌层和基质高,48 s后,病灶信号较宫颈基质高,与宫颈基质之间仍有良好的对比,延时300 s后,15例(75%)病灶边缘呈环形强化,比T2WI可更清楚显示病灶与阴道穹窿、宫颈管的关系,但与T2WI相比病灶范围"缩小"。宫颈癌MR平扫分期准确率为72.2%。平扫和动态增强MR综合分期准确率为88.9%。结论 动态增强扫描对宫颈癌和周围组织的关系显示更好,对小宫颈癌检出更敏感,MR平扫结合动态增强扫描可提高对宫颈癌的定位、定性和分期准确性。
英文摘要:
      Objective To evaluate the diagnostic value of dynamic MR imaging on cervical cancer. Methods Twenty-one patients with cervical cancer proved by biopsy were performed MR examination, and time-intensity curves were derived from dynamic MR imaging of lesions, uterus smooth muscle and cervical stroma. Results Sixteen second after injection of contrast agent, 20 lesions showed hyperintensity, 16(80%) of which showed higher intensities than uterus muscle and stroma. 48 s later, hyperintense lesions were well differentiated from surrounding stroma. 300s later, 15 (75%)lesions showed ring-like enhancement, which clearly demonstrated the anatomic relation of lesions, vaginal fornix, and cervical canal, but the sizes of lesions "shrank". Staging accuracy of lesions based on routine MR was 72.2%, and staging accuracy of lesions based on routine MR and dynamic enhancement was 88.9%, higher. Conclusion Dynamic MR imaging is more sensitive to small lesion, and differentiates lesions from surrounding tissues better. Combination of MR routine scan and dynamic imaging is the best choice.
