目的 初步探讨应用64层螺旋CTA、CTP成像分析大脑中动脉狭窄分级与脑血流灌注的相关性。方法 47例症状性大脑中动脉狭窄患者采用CTP、CTA"一站式"检查。根据CTA成像对大脑中动脉狭窄进行分级。Ⅰ级:管腔面积狭窄率<50%;Ⅱ级:管腔面积狭窄率50%~70%;Ⅲ级:管腔面积狭窄>70%。计算病变侧/健侧CT灌注4项指标rCBF、rCBV、MTT、TTP的比值。结果 Ⅰ级狭窄:11例, 病变侧/健侧CBF、CBV、MTT、TTP比值等于1。Ⅱ级狭窄:29例,病变侧/健侧CBF、CBV、MTT、TTP比值为0.94、1.05、1.35、1.41。Ⅲ级狭窄:7例, 病变侧/健侧CBF、CBV、MTT、TTP为0.58、0.69、1.65、1.64。结论 科学分析CT灌注4项指标能够准确反映大脑中动脉狭窄低灌注状态。脑血流低灌注状态与狭窄的部位、分级和侧支代偿情况密切相关。 |

Objective To discuss the relevance between symptomatic middle cerebral artery (MCA) stenosis and cerebral blood flow with 64-slice CT angiography (CTA) and CT perfusion (CTP). Methods Forty-seven patents with symptomatic MCA stenosis performed CTP and CTA in "one-stop"style examination. Degree of MCA stenosis were classified according to result of CTA: degree Ⅰ: narrow areas <50%; degree Ⅱ :narrow areas 50%~70%; degree Ⅲ: narrow areas>70%. The ratios of side-to-side were measured at hypoperfusion areas in regional cerebral ischemia. Results Degree Ⅰ: 11 patients, CBF, CBV, MTT, TTP mean ratio was 1. Degree Ⅱ: 29 patients, the means ratio of CBF, CBV, MTT, TTP were 0.94, 1.05, 1.35 and 1.41 respectively. Degree Ⅲ: 7 patients, the mean ratio of CBF, CBV, MTT, TTP were 0.58, 0.69, 1.65 and 1.64 respectively. Conclusion Scientific analysis CT perfusion can accurately reflect the hypoperfusion of MCA stenosis. The state of cerebral artery hypotension has significantly relevance with stenosis positions, grade and lateral branch compensation. |
