      目的 探讨氢质子磁共振波谱(1H-MRS)在脑星形细胞瘤中代谢物特点及其与病理分级的相关性。方法 对26例脑星形细胞瘤患者行常规MRI、增强扫描和1H-MRS扫描。经病理证实的低级别星形细胞瘤(LGA)12例、间变性星形细胞瘤(AA)7例和胶质母细胞瘤(GBM)7例,并分成低度恶性组和高度恶性组。用机器自带波谱软件进行分析,检测肿瘤强化区、坏死区、水肿区及对侧正常参照区N-乙酰门冬氨酸(NAA)、胆碱(Cho)、肌酸(Cr)、乳酸(Lac)和肌醇(mI)代谢物浓度,比较两组肿瘤强化区、坏死区、水肿区及各组肿瘤强化区、坏死区、水肿区与对侧正常参照区NAA/Cr、Cho/Cr及NAA/Cho比值差异有无统计学意义。结果 脑星形细胞瘤波谱表现为NAA/Cr和NAA/Cho比值降低,而Cho/Cr比值升高。两组肿瘤强化区、坏死区、水肿区分别与对侧正常参照区NAA/Cr、Cho/Cr及NAA/Cho比值比较均有显著性差异(P<0.05)。强化区Cho/Cr比值在低度恶性组、高度恶性组分别是1.97±0.77、3.11±2.64,坏死区Lac值在低度恶性组、高度恶性组分别是1.18±0.86、3.50±1.39,水肿区Cho/Cr比值在低度恶性组、高度恶性组分别是1.05±0.46、1.37±0.41。强化区Cho/Cr比值、坏死区Lac值和水肿区Cho/Cr比值在低度恶性组与高度恶性组之间均有显著性差异(P<0.05)。结论 结合常规MRI,1H-MRS可提高对脑星形细胞瘤诊断、鉴别诊断及分级的准确性,并提供一个定量分析的方法。

      Objective To study 1H-MR spectroscopic pattern of brain astrocytomas and report characteristic spectral patterns that may be helpful in grading astrocytomas. Methods Twenty-six cases of astrocytomas with pathologic confirmation underwent routine MRI, enhanced and 1H-MR spectroscopy examination. They were divided into benign tumors' group and malignant tumors' group: 12 patients of low-grade astrocytomas (LGA), 7 patients of anaplastic astrocytomas (AA) and 7 patients of glioblastomas multiforme (GBM). N-acetylaspartate (NAA), choline (Cho), creatine (Cr), lactate (Lac) and my-inosine (mI) values were measured in the regions of enhancement, necrosis, edema, and corresponding normal brain. The corresponding the NAA/Cr, Cho/Cr and NAA/Cho ratios were obtained from both the regions of enhancement, necrosis, edema, and corresponding normal brain of each group. Results Decreased NAA and increased Cho were observed in the astrocytomas. In the regions of enhancement, necrosis and edema, the NAA/Cr, Cho/Cr and NAA/Cho ratios changed significantly when comparing with that of normal control. There was a significant difference between benign group and malign group of the Cho/Cr ratios in the region of enhancement (P<0.05). A significant difference of the Lac in the region of enhancement between benign group and malign group (P<0.05) was also observed. A significant difference of the Cho/Cr ratios existed in the region of edema in the two groups of astrocytoma (P<0.05). Conclusion Proton MR spectroscopy can improve diagnostic accuracy of astrocytoma and provide a helpful technique of quantitative analysis for grading astrocytoma on MR images.
