      目的 探讨应用PLE实现功能性肺叶切除(FPLT)后对靶肺及非靶肺的病理变化,评价其可行性。方法 健康犬18只,随机分成2组,A组12只,B组6只。A组行靶肺支气管肺泡灌注平阳霉素-碘化油乳剂(PLE)及封堵器堵塞靶支气管,B组为对照组。A组分别于14天、28天、56天各处死4只取肺组织,B组各处死2只,测定肺纤维化的程度,运用图像分析仪测量肺动脉靶肺肺小动脉管壁面积指数(AI)和中膜厚度指数(TI)增高。结果 FPLT术后A组动物组织病理学显示靶肺呈纤维化改变,肺小动脉管壁增厚,管腔部分闭塞;非靶肺未见明显纤维化,无血管重构现象出现。结论 通过靶肺支气管肺泡灌注PLE后靶支气管封堵器堵塞可达到功能性肺叶切除术目的。靶肺肺小动脉出现血管重构现象,有利于术后通气-血流比失调的调节,对肺循环无明显不良影响。

      Objective To investigate the pathological findings in dogs of functional pulmonary lobectomy (FPLT), and to evaluate influence on the pulmonary circulation after FPLT and it's feasibility. Methods Eighteen healthy dogs were divided into group A (n=12) and group B (n=6) randomly. Group A: pingyangmycin-lipiodol-emulsion (PLE) was injected to the target lung, and the target bronchia was obstructed with broncho-occluder by catheter. Group B was normal group. Four dogs of group A and two dogs of group B were sacrificed on 14th, 28th and 56th day respectively, and the lungs were harvested for pathological examination. The degree of pulmonary fibrosis was measured. The measurement of ratio of vessel wall area (AI) and thickness index (TI) were accomplished by using the microscopic image analysis program. Results It was showed histopathologically that pulmonary fibosis with different degree in target lung of group A. The thickness in middle and small pulmonary arterial wall increased and the cavity became narrow, no fibrosis and no vascular remodeling in nontarget lung after FPLT. Conclusion FPLT could be obtained after PLE was injected to the target lung, and target bronchia obstructed with broncho-occluder by catheter. The increased thickness in middle and small pulmonary arterial wall the cavity narrowing can adjust the ratio of ventilation and blood flow. It was no influence to the pulmonary circulation after FPLT.
