| 目的 探讨蛛网膜颗粒在薄层CT及相应后处理成像中的表现。方法 观察100例患者的原始薄层CT图像、容积再现(VR)和最大密度投影(MIP)后处理图像,研究蛛网膜颗粒的发现率、分布部位、大小及其与静脉窦流入静脉的关系。结果 100例中66例发现蛛网膜颗粒, CT上表现为静脉窦内圆形或类圆形低密度充盈缺损;66例共发现175个蛛网膜颗粒,其中横窦90个,上矢状窦77个,直窦5个,窦汇2个,下矢状窦1个。92%横窦内蛛网膜颗粒(83／90)位于浅表静脉汇入横窦处;22例可见静脉直接进入蛛网膜颗粒内。横窦引流优势和非优势对蛛网膜颗粒的发现率未见明确影响。上矢状窦蛛网膜颗粒大多数位于顶后方,多为小而多发。年龄与蛛网膜颗粒数目及大小之间均未见明显相关性。结论 在薄层CT图像上蛛网膜颗粒是常见的正常表现,认识蛛网膜颗粒的正常表现可以与发生于静脉窦的疾病相鉴别。 |

| Objective To determine normal appearances of arachnoid granulations in thin-sliced CT imaging and VR, MIP imaging. Methods Contrast-enhanced CT imaging and CT angiography of 100 consecutive patients were investigated. The incidence, location, size of arachnoid granulations (AGs) and their relationship with adjacent veins were recorded. Results AGs were found in 66 patients. AGs appeared as oral filling defect in venous sinus at contrast-enhanced CT imaging. Of the total 175 AGs, 90 AGs were found in transverse sinus, 77 in superior sagittal sinus, 5 in straight sinus, 2 in torcular Herophili, 1 in inferior sagittal sinus. 92% AGs in the transverse sinus were exactly at the termination of superficial vein at transverse sinus. Superficial vein directly entered AGs were found in 22 cases. Dominant and none-dominant transverse sinus had no influence on incidence of AGs. The most locations for AGs in superior sagittal sinus were at the posterior and superior of the sinus, always multiple and small. AGs had no significant correlation between age and number of AGs, or age and size of AGs. Conclusion AGs are normal structures that are frequently found in the cerebral venous sinuses on thin-sliced CT images. AGs can be differentiated from pathological processes regarding their incidence and normal appearance. |
