| 目的 利用下肢束带法实现对健康成年人脑动脉自动调节功能的无创、定量测定,并初步探讨此方法的临床实用性。方法 选择72例由经颅多普勒超声(TCD)和颈部双功能超声排除颅内、外血管病变的健康志愿者作为研究对象,观察放松双股束带后10 s内平均动脉压和双侧大脑中动脉(MCA)血流速度的变化,通过比较二者对时间的变化率,得出健康成年人的脑动脉自动调节指数(ARI)。结果 健康受试者在放松双股束带后的双侧MCA血流速度在短暂下降后,迅速回升至基线水平,同步血压记录也呈现先下降后上升的趋势,但上升速率明显慢于MCA血流速度。利用TCD8软件分析得出健康成年人的ARI值为5.25±1.39。ARI与受试者年龄、平均动脉压之间无明显关系(P>0.05)。结论 本研究表明利用TCD测定MCA流速、无创记录血压,可以便捷、定量的评价脑动脉的自动调节功能。 |

| Objective To determine the dynamic cerebral autoregulation in healthy adults using thigh cuff technique noninvasively. Methods We used thigh cuffs technique to estimate dynamic cerebral autoregulation in 72 healthy adults who were excluded from intra-and extra-cranial cerebral vascular disease by transcranial Doppler and carotid Duplex. After a stepwise fall in arterial blood pressure, we determined the rate of rise of middle cerebral artery blood velocity compared with that of arterial blood pressure. Results Autoregulatory index in the 141 middle cerebral arteries in 72 healthy adults was 5.25±1.39. There was no relationship between autoregulatory index and age or mean arterial blood pressure. Conclusion Cerebral autoregulation might be continuously and conveniently evaluated by measuring middle cerebral artery velocity and artery blood pressure simultaneously using thigh cuff technique. |
