| 目的 探讨随访时间对实时三维超声心动图(RT3DE)立体显示人工瓣效果的影响。方法 在术后5天~12年,对80例接受人工瓣膜置换的患者进行了125次RT3DE成像。结果 在术后5天~2个月(含2个月),图像质量佳者占75.0%(63/84),图像质量差和受呼吸影响者占25.0%(21/84);在2个月以上~12年,图像质量佳者占97.6%(40/41),图像质量差和受呼吸影响者占2.4%(1/41)(P<0.01)。在术后5天~1年(含1年),房室瓣位机械瓣瓣环清晰者占86.2%(50/58),瓣环不清者占13.8%(8/58);在1年以上~12年,瓣环清晰者占11.1%(1/9),不清者占88.9%(8/9)(P<0.001)。结论 随着术后随访时间延长,RT3DE图像质量逐渐得到改善;清晰显示房室瓣机械瓣环的比例逐渐降低,考虑是内膜爬行完全覆盖人工瓣环表面所致。 |

| Objective To evaluate the effect of postoperative follow-up duration on prosthetic valves on real-time three-dimensional echocardiography (RT3DE). Methods Totally 80 patients with prosthetic valves underwent RT3DE for 125 times between five days and twelve years during postoperative follow-up period. Results From five days to two months (inclusive) during the postoperative period, the quality of three-dimensional images was high in 75%(63/84), and poor or affected by respiratory motion in 25% (21/84); while from two months to twelve years, high in 97.6%(40/41), and poor or affected by respiratory motion in 2.4% (1/41) (P<0.01). From five days to one year(inclusive), the mechanical prosthetic atrioventricular annulus was clear in 86.2% (50/58), and unclear in 13.8% (8/58); while from one year to twelve years, clear in 11.1%(1/9),and unclear in 88.9% (8/9)(P<0.001). Conclusion With the prolongation of the follow-up duration, the quality of three-dimensional images will be improved, whereas the clarity of the mechanical prosthetic atrioventricular annulus will be decreased gradually. The cause of this phenomenon is due to the endocardial growing over to the surface of the prosthetic atrioventricular annulus. |
