      目的 用ROC曲线及Logistic回归模型综合评价圆形指数在浅表淋巴结良恶性鉴别诊断中的价值。方法 对168例经穿刺病理及随访证实的浅表淋巴结超声测量的圆形指数进行分析,绘制ROC曲线并计算曲线下面积,并通过圆形指数在二分类Logistic回归模型中的贡献及分类判对率探讨其在良恶性鉴别诊断中的价值。结果 圆形指数判断淋巴结良恶性的ROC曲线下面积结果与其t检验结果一致。然而,虽然良恶性淋巴结的圆形指数有非常显著性差异,1.985 vs 1.504(P<0.001),但其ROC曲线下面积只达到0.770±0.039,当取值为大于1.691时,诊断为良性淋巴结的敏感性和特异性分别为70.7%和72.0%。二分类Logistic回归分析显示,圆形指数经前进法逐步回归后引入方程,在模型中对良恶性判断有明显的影响(圆形指数的偏回归系数为-2.550±0.601,χ2值为46.515,模型分类判对率为74.4%,优势比为0.078)。结论 超声测量的圆形指数是简便判断浅表淋巴结良恶性的重要指标之一,有一定鉴别诊断应用价值。
英文摘要:
      Objective To evaluate the value of roundness index in differential diagnosis of lymph nodes by receiver operating characteristic (ROC) curves and model of Logistic regression. Methods Analyzed the roundness index of superficial lymph nodes measured by ultrasound in 168 patients proved by pathology, we drawn ROC curve and calculated the areas under curves by the primary data of roundness index.We also evaluated the value of roundness index in differential diagnosis of lymph nodes by its contribution and the percentage of correct class through the model of binary Logistic regression analysis. Results The results of predicting the nature of lymph nodes by the area under curves of ROC were in accordance with the results of t-test. However, although the roundness index of benign and malignant lymph nodes had significant statistical differences (1.985 vs 1.504, P<0.001), the results of ROC demonstrated that the area under curves only reached 0.770±0.039. When the roundness index was larger than 1.691, the sensitivity and specificity was 70.7% and 72.0%, respectively. The results of binary Logistic regression analysis displayed that roundness index was inducted in model through the forward stepwise, and its influence on estimation of benign and malignant lymph nodes was the greater one (its unstandardized coefficients was -2.550±0.601, the value of χ2 was 46.515, correct class was 74.4%, odds ratio was 0.078). Conclusion The roundness index of superficial lymph nodes measured by ultrasound is a simply and important index in predicting the nature of lymph nodes, and has great practical value in differential diagnosis.
