目的 通过观察单侧脑血管狭窄或闭塞患者对侧小脑血流及葡萄糖代谢的变化,了解交叉性小脑神经机能联系不能(CCD)与原发脑缺血组织的关系。方法 对33例单侧脑血管狭窄或闭塞患者的脑血流灌注显像及脑葡萄糖代谢显像结果进行对比分析,当小脑不对称指数(AI )大于0.1时诊断为CCD。结果 脑血流灌注显像CCD的发生率为42%,脑葡萄糖代谢显像CCD的发生率为73%(χ2=6.203, P=0.013)。两种检查方法中CCD患者的原发缺血部位的分布无显著差异(χ2=2.600, P=0.761)。发生CCD的对侧小脑的低血流灌注或低葡萄糖代谢在亚急性期和慢性期无显著差异(ECD:t=-0.522,P=0.608;FDG: t=1.751, P=0.093)。结论 在脑血流灌注显像中出现CCD时最常受累的部位为顶叶和基底节,而在脑葡萄糖代谢显像中最常受累部位为顶叶,当发生CCD时脑葡萄糖代谢显像更敏感。 |

Objective To evaluate the relationships between crossed cerebellar diaschisis (CCD) and cerebral lesion by observing the varieties of cerebellar glucose metabolism and cerebellar blood flow perfusion in patients with single-side cerebral vessel stenosis or occlusion. Methods A total of 33 patients with single-side involved and without lesions on brain MR images underwent cerebral metabolish image and cerebral blood flow perfusion image, and the results were compared. CCD was defined as AI for cerebellum >0.1. Results The incidence rate of CCD in patients in cerebral blood flow perfusion image was 42%, 73% in cerebral glucose metabolism image(χ2=6.203, P=0.013). The impaired area in patients with CCD were no significant in the cerebral glucose metabolism image and cerebral blood flow perfusion image(χ2=2.600, P=0.761). There were no statistically significant change in cerebellar hypoperfusion or hypometabolism between the subacute stage and chronic stage, as assessed with 99Tcm-ECD SPECT and 18F-FDG coincidence SPECT. Conclusion CCD frequently were observed in patient with parietal lobes lesion and basal ganglia lesion in cerebral blood flow perfusion image and parietal lobes lesion in cerebral glucose metabolism image. Cerebral glucose metabolism image is more sensitive than cerebral blood flow perfusion image in diagnosing CCD. |
