| 目的 探讨肾癌螺旋CT三期增强扫描征象,提高诊断水平。方法 回顾性分析51例经手术病理证实的肾癌多期螺旋CT扫描资料。结果 临床典型肾癌多为中等大小(3 cm<<7 cm)肿块,密度不均(实性16/51,囊实性24/51,囊性11/51),皮质期明显强化,实质期迅速下降。小肾癌(≤3 cm)多突出于肾轮廓外,中等(3 cm<<7 cm)和大肾癌(≥7 cm以上)多贯通肾实质,与正常肾实质分界形态多为弧形缺损。肾周间隙改变多提示突破包膜,大肾癌易出现包膜周围血管增粗(5/12)。三期增强易显示肾周淋巴结(14/17),静脉癌栓(15/15)及肾盂受侵(28/32)。结论 螺旋CT三期增强扫描可全面显示肾癌病变征象及范围,肾癌仍有值得探讨的有价值征象。 |

| Objective To investigate the signs of renal cell carcinoma (RCC), and improve the imaging diagnosis of RCC with tri-phase enhancement helical CT scanning. Methods The plan and multi-phasic incremental helical CT scan datum of 51 patients surgical-pathologically proved renal cell carcinoma were analyzed retrospectively. Results In clinic, typical renal cell carcinoma were in middle size (3 cm<<7 cm) and heterogeneous density, strong enhanced in cortical phase, and rapidly faded away in the following parenchyma phase. Small renal cell carcinoma (≤3 cm) protrude out of contour of the kidney mostly; most middle (3 cm<<7 cm) and large size (≥7 cm) RCC breakthrough the parenchyma of kidney, and the borderline of the normal kidney and the carcinoma resemble a defect shape of arc. Perirenal space abnormalities in RCC were some cues that RCC breakthrough the amicula of kidney,and larger RCC showed broaden blood vessels around amicula of kidney in many cases(5/12). Lymphonode, carcinomatous embolus of renal vein and IVC, and pelvis invasion was well showed in tri-phase incremental helical CT scan. Conclusion Tri-phasic enhancement helical CT scanning well demonstrate the features of the lesions, the way show some other valuable CT signs of RCC. |
