      目的 探讨实时三维超声心动图技术对无顶冠状静脉窦综合征的诊断价值。方法 应用实时三维超声心动图获取10例无顶冠状静脉窦的实时三维和全容积三维数据库,切割图像并观察冠状静脉窦顶壁缺损的形态特征或左上腔静脉回流位置等,其中9例与手术结果对照,1例与CT检查结果对照。结果 10例无顶冠状静脉窦中,Ⅰa型2例,Ⅰb型2例,Ⅱb型2例,Ⅲb型4例。实时三维超声可显示Ⅱ型和Ⅲ型无顶冠状静脉窦的顶壁缺损的完整形态及毗邻关系,2例Ⅰa型可显示永存左上腔静脉的回流部位及入口形状,2例Ⅰb型实时三维超声与二维超声均不能提供有价值信息。结论 实时三维超声可直视无顶冠状静脉窦综合征Ⅱ型和Ⅲ型的顶壁缺损全貌以及Ⅰa型永存左上腔静脉的回流部位等,可提供二维超声心动图所不能获得的信息。

      Objective To explore the value of real-time three-dimensional echocardiography (RT-3DE) in visualization of pathological structures of unroofed coronary sinus syndrome (UCSS). Methods Ten patients with UCSS were examined with RT-3DE, both "Live 3D"and "Full volume" images were rendered and cropped in acquiring best visualization of three-dimensiona1 heart anatomy. Three-dimensional echocardiographic (3DE) images were compared with surgical findings or computed tomography (CT). Results Ten cases of UCSS included 2 cases of Ⅰa type, 2 cases of Ⅰb type, 2 cases of Ⅱb type, 4 cases of Ⅲb type. RT-3DE provided unique views of coronary sinus septal defect (CSSD) in Ⅱ, Ⅲ type and the site of persistent left superior vena cava (PLSVC) draining into the left atrial (LA) side in Ⅰa type, allowing a complete assessment of their circumference and spatial orientations to other anatomic structures in Ⅱ and Ⅲ type. Conclusion RT-3DE permitted a direct visualization of coronary sinus defect in Ⅱand Ⅲ type of UCSS and draining site and orifice of PLSVC in Ⅰa type of UCSS. RT-3DE could provide additional information of pathological structures of UCSS superior to two-dimensional echocardiography.
