| 目的 初步探讨供肝移植前超声快速评价供肝脂肪变性的价值。方法 用传统二维超声诊断法及中场灰度值测量法对25例供肝移植前快速评定有无脂肪变性,并与脂肪含量测定及病理分型做对比分析。 结果 以脂肪含量测定为金标准,传统二维超声诊断法及以供肝中场灰度值≥40诊断脂肪肝的敏感性、特异性及准确性分别为55.6%、100％、84％及88.9%、81.2%、85%。结论 供肝移植前,用超声快速评价供肝脂肪变性具有较高的特异性,以中场灰度值补充诊断,可在一定程度上提高敏感性。 |

| Objective To evaluate the value of ultrasonography in assessment of hepatic steatosis of the liver graft before transplantation. Methods The ultrasonographic results of fatty deposit in 25 liver grafts were confirmed by pathological diagnosis with Sudan Ⅲ staining and lipid determination with Soxhlet's extraction procedure before transplantation. Traditional 2-D ultrasonography and gray-scale density measurement in middle field were performed in each case. Results Compared with adipose determination, the sensitivity, specificity and accuracy were 55.6%, 100%, 84% in real-time 2-D ultrasonography and 88.9%, 81.2%, 85% in gray-scale density measurement in middle field respectively. Conclusion The specificity of ultrasonography in determining hepatic steatosis of the liver graft before transplantation is high. The sensitivity can be improved by adding gray-scale density measurement in middle field. |
