目的 评价口服葡萄糖酸亚铁糖浆溶液在磁共振胰胆管成像(MRCP)中的应用价值。方法 实验部分:将不同浓度医用硫酸钡和葡萄糖酸亚铁糖浆两种溶液装入不同试管中,行MR T2WI和single-shot扫描,改变TE时间,观察信号强度和不同反转时间(TI)信噪比的变化。临床试验:把100位患者随机分成两组行不服对比剂和口服钡或铁剂5、10、15 min分别进行检查,观测胃肠道图像信噪比变化及以胰胆管解剖结构为对象的MRCP图像的改善情况。结果 60%硫酸钡和14%葡萄糖酸亚铁溶液将水信号基本抑制;当TI时间为130 ms左右时,铁剂溶液的信噪比强度降低明显;钡剂和铁剂的信号随TE时间的延长而降低。服用钡剂和铁剂后胃内信噪比平均值降至1.043±0.456, P<0.001;服用对比剂后十二指肠的平均信噪比降至1.930±2.315, 1.933±2.320, P<0.001。口服对比剂前与口服对比剂后10 min MRCP成像比较,对胆总管、胰管、胆囊结构的显示,钡剂和铁剂均具有显著性差异(P=0.012,0.010,0.014; P=0.013, 0.011, 0.015);与口服对比剂后15 min成像比较,对胆总管、胰管、胆囊结构的显示,钡剂和铁剂均具有显著性差异(P=0.006, 0.018, 0.024; P=0.007, 0.019, 0.025),但后者图像更佳。结论 口服阴性胃肠道对比剂(V/V)14%葡萄糖酸亚铁糖浆溶液约500 ml后,15 min进行MPCP检查,将获得极佳的MRCP图像。葡萄糖酸亚铁溶液是MRCP成像时良好的胃肠液抑制剂,使MRCP图像更加清楚,使对胰胆系疾病的定位、定性及定量诊断更加准确。 |

Objective To evaluate the clinical value of ferrous gluconate syrup in magnetic resonance cholangiopancreatography (MRCP). Methods Different test tubes with different concentrations of ferrous gluconate syrup were scanned by using MR T2WI and single-shot MRCP. 100 patients were randomly divided into two groups. MRCP was performed in 50 patients before and 5, 10, and 15 min after taking barium sulfate suspension, and in other 50 patients before and 5, 10, and 15 min after taking ferrous gluconate syrup. The visualization of the common bile duct, pancreatic duct, and gallbladder was evaluated. Results 60% (W/V) barium sulfate suspension and 14% (V/V) ferrous gluconate syrup were the best for inhibiting the high signal from the water in the gastrointestinal tract in each group. The signal of barium sulfate suspension and ferrous gluconate syrup were reduced with prolonging time of echo (TE). After administration, the CNR of stomach was decreased to 1.043±0.456 (P<0.001), and the CNR of duodenum was decreased to 1.930±2.315, 1.933±2.320 (P<0.001). Significant difference in displaying the common bile duct, pancreatic duct and gallbladder existed between the images before and after drinking negative gastrointestinal contrast media 10 min (P=0.012, 0.010, 0.014; P=0.013, 0.011, 0.015), and after drinking negative gastrointestinal contrast media 15 min (P=0.006, 0.018, 0.024; P=0.007, 0.019, 0.025). Conclusion Diluted ferrous gluconate syrup solution (14%) 500 ml can be used as negative gastro-duodenal contrast agent to suppress the high signal background. MR scanning performed 15 min after taking the diluted ferrous gluconate syrup solution can provide high quality MRCP images. So it will improve the accurate rate of diagnosis in pancreatic-biliary duct. |
