目的 描述并评价全肝3D动态MRI测量肝脏灌注成像(PI)参数的方法。方法 50个装有不同浓度Gd-DTPA的生理盐水溶液(浓度范围0~6 mM)试管模型,使用3D-GRE序列,选择不同的翻转角(10°、15°、25°、45°); 10只兔肝VX2肿瘤模型,采用不同对比剂浓度(0.05、0.1、0.2 mmol/kg)进行动态全肝MR PI扫描,找出最佳扫描参数。灌注成像序列块厚60 mm,扫描141~150 s,共40~45次扫描,扫描开始后10 s注射Gd-DTPA。通过体外试管实验得出的数据,将时间-信号强度曲线转换为时间-浓度曲线。测量、比较VX2肿瘤区域与正常肝脏的开始强化时间、最大对比剂浓度、达到最大浓度的时间、平均早期强化斜率及强化曲线类型。结果 采用25°翻转角、0.05 mmol/kg的对比剂浓度得到的PI曲线最佳。肿瘤区域的开始强化时间、最大对比剂浓度和平均早期强化斜率分别为(16.6±1.94)s、(0.59±0.11)mM及(0.0388±0.0086) mmol/L/S,与正常肝脏组织 比较有显著性差异(P=0.0167, 0.0362,0.0059)。结论 采用3D-GRE序列、25°翻转角、0.05 mmol/kg的对比剂浓度能得到高质量的全肝动态MR灌注成像,可用于分析肝实质与肝内病灶不同的微血管动力差异。 |

Objective To describe and validate the use of dynamic whole-liver 3D MRI for the noninvasive assessment of hepatic perfusion imaging parameters. Methods The signal from 3D-GRE sequences using different flip angles (10°, 15°, 25°, 45°) was calibrated in vitro with 50 tubes filled with various gadolinium concentrations (0-6 mM). Dynamic whole-liver 3D images in 10 rabbits (all male, kg) with VX2 tumor in liver (diameter of 2-4 cm) were obtained with 3D GRE sequence of 25°flip angle. 40-45 dynamic scans with each 60 mm slab were obtained during 141-150 s, and the intravenous bolus administrations of various concentration (0.05, 0.1, 0.2 mmol/kg) of Gd-DTPA started at the 10th second after scan beginning. Time-intensity curves were described and were converted to time-concentration curves through the test in vitro. The enhancement beginning time, the max concentration of Gd-DTPA, the time to the max concentration, the enhancement average slope and the curve types were measured, and the differences of these measurements between the tumor and normal liver parenchyma were compared in respectively. Results The most effective perfusion curve can be obtained with 25° flip angle and 0.05mmol/kg of Gd-DTPA. The difference of parameters in the t-test between the tumor (the enhancement beginning time s; the max concentration mM; the enhancement average slope mmol/L/S) and normal liver parenchyma (the enhancement beginning time s; the max concentration of Gd-DTPA mM; the enhancement average slope mmol/L/S) was statistically significant (P=0.0167, 0.0362, 0.0059). Conclusion Using 3D-GRE sequence with 25° flip angle and the intravenous bolus administration of 0.05 mmol/kg of Gd-DTPA, the most effective perfusion curve of whole-liver can be obtained, which will be helpful in evaluating the microvessel difference between tumor and normal liver parenchyma. |
