      目的 通过对星形细胞瘤瘤周水肿区最大相对脑血流容积( rCBV) 比值与MMP-9 免疫组化对照研究,旨在探讨星形细胞瘤磁共振灌注成像( PWI) 与肿瘤侵袭性和肿瘤分级的相关性。方法 对28 例伴有瘤周水肿的星形细胞瘤术前采用对比剂首过动态增强自旋回波2平面回波成像序列进行PWI 扫描,以灌注原始图像重建出rCBV 图,依据rCBV 图计算出瘤周水肿区最大rCBV 值,再与对侧大脑相应部位rCBV 值相比,高于对侧正常白质的为瘤周水肿高灌注组,低于对侧正常白质的为瘤周水肿低灌注组,得出瘤周水肿区最大rCBV 比值。术后取肿瘤标本进行MMP-9 免疫组化染色,计算每一标本的MMP-9 表达程度。分析28 例瘤周水肿区最大rCBV 比值与MMP-9 表达的相关性。结果 瘤周水肿高灌注组17例,相应MMP-9 标记指数为(52. 91 ±14. 33) %;瘤周水肿低灌注组11 例,MMP-9 标记指数为(29. 86 ±13. 50) %。比较两组间的肿瘤MMP-9 标记指数的差异,行×列表χ2 检验,具有统计学意义( P < 0. 01) 。结论 星形细胞瘤瘤周水肿最大rCBV 比值与MMP-9 表达存在相关性,瘤周水肿区最大rCBV 比值增大侵袭性增强,肿瘤病理分级相应增高。灌注成像可用于活体上评价星形细胞瘤的侵袭性。
英文摘要:
      Objective To investigate the relationship between MR perfusion weighted imaging ( PWI) and tumor invasion.Methods MR PWI was performed preoperatively on 28 patient s. Color maps of rCBV were reconst ructed f rom the perfusiondata. The maximum rCBV ratio in the peritumoral edema was calculated for each lesion on the rCBV maps. The expressionlevels of MMP-9 were also obtained. The maximum rCBV ratios in the peritumoral edema of 28 patient s were compared withMMP-9 expression. Results The maximum rCBV ratio of the peritumoral edema was related to the MMP-9 expression.There was significant difference in the MMP-9 labeling index between groups with hyper-and hypo-perfusion in peritumoraledema ( P < 0. 01) . Conclusion The maximum rCBV ratio of the peritumoral edema was associated with MMP-9 expression.MR PWI is feasible for evaluating the invasion and pathologic grading of ast rocytomas.
