目的 对比磁共振扩散加权成像(DWI) 、灌注加权成像( PWI) 和CT 灌注成像(CTPI) 评价急性脑静脉闭塞模型的价值。方法 家猫18 只,随机分为6 小组,术后1 、3 、6 、12 、24 、48 h 各3 只。采用开颅上矢状窦穿刺注射液体栓塞剂———醋酸纤维素聚合物(CAP) 制备急性脑静脉闭塞动物模型,对各组模型行DWI、PWI 和CTPI 检查,对比MR、CT 对病灶检出率、脑实质损害容积及程度判断的一致性。结果 DWI 发现10 只猫脑实质出现19 个异常信号,PWI 发现13 只猫脑实质内23 个异常灌注区,CTPI 发现12 只猫脑实质内20 个异常血流灌注区。PWI 平均通过时间(MTT) 参数图与CTPI局部脑血流量( rCBF) 参数图对病灶容积的测量差异无统计学意义( t = 1. 54 , P > 0. 05) 。PWI 和CTPI 局部脑血容量( rCBV) 图对脑实质损害程度判断一致性好( Kappa = 0. 69 , P < 0. 01) 。结论 DWI、PWI 与CTPI 对急性脑静脉闭塞脑实质损害的判断均能提供重要信息,具有较好的一致性。 |

Objective To compare the value of MR diffusion weighted imaging (DWI) , perfusion weighted imaging( PWI) , and CT perfusion imaging (CTPI) in evaluating a model of acute cerebral venous occlusion. Methods Eighteen cat swere randomly divided into 6 groups , including 3 cat s in 1 , 3 , 6 , 12 , 24 and 48 h af ter operation , respectively. The modelswere performed by injection of the cellulose acetate polymer (CAP) solution into the superior sagit tal sinus ( SSS) . DWI ,PWI and CTPI were continually performed to observe the brain parenchymal injury. Detection of lesions , lesion volumes andthe degree of brain parenchymal injury were compared. Results DWI detected 19 lesions in 10 cat s. PWI detected 23 abnor-mal perfusion areas in 13 cat s. CTPI detected 20 lesions in 12 cat s. The lesion volumes on PWI mean t ransit time (MTT)maps showed no significant difference with that on CTPI regional cerebral blood flow ( rCBF) ( t = 1. 54 , P > 0. 05) . Theevaluation of the degree of brain parenchymal injury on PWI and CTPI regional cerebral blood volume ( rCBV) showed goodconsistency( Kappa = 0. 69 , P < 0. 01) . Conclusion DWI and PWI have good consistency with CTPI in judging the brain pa-renchymal lesions in acute cerebral venous occlusion. |
