| 目的 探讨螺旋CT 各种征象(包括病灶厚度、大小、强化特点、浸润深度、淋巴结转移和TNM 分期) 与手术病理之间的关系。方法 本组对57 例胃癌行低张水充盈螺旋CT 三期增强扫描,并与手术病理对照。结果 螺旋CT 表现病灶厚度、大小与手术病理的浆膜侵犯、淋巴结转移和TNM 分期均相关(P<0.05) ;螺旋CT 的强化特点与手术病理的淋巴结转移相关(P<0.05) ,与浆膜侵犯及TNM 分期不相关。螺旋CT 对胃癌浸润深度、淋巴结转移和远处转移的准确性分别为82. 46 %、78. 95 %和100 % ,均与手术病理一致(P<0.05) 。结论 胃癌螺旋CT 征象与手术病理之间具有相关性,可依据其影像表现特点推测出胃癌的病理学特性,从而指导手术及判断预后。 |

| Objective To evaluate the correlation between spiral CT features and operative pathology , all kinds of spiralCT features involved in tumor thickness , size , enhancement pattern , the depth of invasion , lymph node metastasis and TNMstaging. Methods A total of 57 patient s with established diagnosis of gast ric cancer were evaluated with t riphasic spiral CT with water filling method and hypotonic agent . Spiral CT findings were compared with operative pathologic staging. ResultsSpiral CT sickness and size of gast ric cancer were correlated with operative pathologic serosal invasion , lymph node metas2tasis and TNM staging (P<0.05). Spiral CT enhancement of gast ric cancer was correlated with operative pathologic lymphnode metastasis (P<0.05) , no correlated with serosal invasion and TNM staging. The accuracy of the depth of invasion ,lymph node metastasis and distant organ metastasis of spiral CT in gast ric cancer were 82. 46 % , 78. 95 % and 100 % , corre2lated with operative pathology (P< 0. 05). Conclusion There was correlation between spiral CT features and operative pa-thology (P<0.05). The preoperative pathologic evaluation with t riphasic spiral CT is useful in directing clinical therapy of gast ric cancer , giving help to direct operative option , constitute therapeutic project and evaluate prognosis. |
