目的 探讨多巴酚丁胺负荷超声(DSE)结合心肌背向散射积分(IBS)检测存活心肌的临床应用价值。方法 冠心病组:22例患者,经冠状动脉定量造影术(QCA)确诊为冠心病(CAD),并进一步行经皮冠脉介入治疗术(PCI),术前1周内行DSE(5,10,20,40 μg/kg·min)检查,于静息、DSE每级负荷5 min采集全心尖切面(长轴、二腔、四腔)IBS图像,术后3个月复查二维超声和IBS检查。对照组:18例患者,QCA结果正常,行2DE,DSE,IBS检查。根据CVIB在DSE过程中的变化特点,探索CVIB最大变化率K值定义存活心肌,并检测K值识别存活心肌的敏感性、准确性,与单纯DSE进行比较。结果 与单纯DSE 相比,DSE结合IBS的K值检测存活心肌敏感性、准确性明显提高(90.0% vs 73.3%;90.4 %vs 77.4%、P<0.05~0.01)。 结论 DSE结合IBS技术较单纯DSE使用能提高检测存活心肌的敏感性和准确性。 |

Objective To explore the clinical value of dobutamine (Dob) stress echocardiography (DSE) combined with integrated backscatter (IBS) for identifying viable myocardium in patients with coronary artery disease (CAD). Methods In CAD group, twenty-two patients who were diagnosed CAD by quantitative coronary artery (QCA), percutaneous coronary intervention (PCI) was performed further. DSE(5, 10,20,40 μg/kg·min)were performed one week before PCI. IBS images at rest and every grade infusion of Dob 5 minutes were obtained at total apical views (Lax, 2-ch, 4-ch). Two-dimensional echocardiography and IBS were examined after three months of PCI again. Eighteen patients whose Results of the quantitative coronary artery were normal underwent the examinations of 2-DE, DSE, IBS in control group. The maximum changing rate of CVIB (K value) during DSE was defined as the criterion of viable myocardium, then the sensitivity and accuracy were compared between DSE and DSE combined with IBS. Results The sensitivity, accuracy of DSE combined with IBS were significantly higher than those of single DSE ( sensitivity: 90.0% vs73.3%,accuracy: 90.4% vs 77.4%, respectively P<0.05-0.01). Conclusion Identifying viable myocardium by DSE combined with IBS has higher sensitivity, accuracy than those of single DSE. |
