目的 初步探讨了应用扩散加权成像(DWI)测量肾脏表观弥散系数(ADC)值的可重复性和临床价值。方法 对10例正常志愿者和40例不同肾脏疾病 患者行DWI检查,为了研究肾脏DWI检查的可重复性,对10例正常志愿者间隔一天后按照相同的序列和参数行第二次DWI检查,分别测量低b值(b1=0 s/mm2、b2=500 s/mm2)和高b值(b1=0 s/mm2、b2=800 s/mm2)DWI图像的肾脏ADClow和ADChigh值,采用Pearson's相关系数和Bland-Altman法分析肾脏ADC值的可重复性。结果 ①Bland-Altman分析表明两次扫描肾脏ADC值的可重复性很好,低b值(0、500 s/mm2)时两次扫描的肾脏ADC差值为-0.0135 mm2/s ,相关系数r=0.897。高b值(0、800 s/mm2)时两次扫描的肾脏ADC差值为-0.0385 mm2/s ,相关系数r=0.773。②肾脏ADClow值明显高于ADChigh值(t=21.462, P<0.001)。③对照组、肾动脉狭窄组、肾积水组和慢性肾病组肾脏ADClow分别为(2.82±0.020)×10-3,(2.42±0.031)×10-3,(2.41±0.063)×10-3和(2.31±0.037)×10-3 mm2/s;ADChigh分别为(2.36±0.022)×10-3,(2.10±0.032)×10-3,(2.15±0.061)×10-3和(1.98±0.043)×10-3 mm2/s,三个疾病组的ADC值均明显低于正常对照组。结论 肾脏DWI可以用来评价肾脏疾病和功能。 |

Objective To evaluate the reproducibility and value of MR diffusion-weighted imaging (DWI) in the measurement of renal apparent diffusion coefficient (ADC) value. Methods Ten healthy volunteers and 40 patients underwent transverse echo-planar DWI. The following renal ADC values of low b values (b=0, 500 s/mm2 and high b values (b=0, 800 s/mm2)were calculated respectively. To assess reproducibility, the same DWI protocols were performed in ten volunteers twice. Results were compared using Pearson's correlation coefficient and Bland-Altman plots. Results ① The mean measurement errors of ADC values between both scans were -0.0135 mm2/s for low b value and -0.0385 mm2/s for high b value. Pearson's correlation coefficients between both scans were 0.897 (ADClow) and 0.773 (ADChigh) respectively. ②In all volunteers and patients, ADClow of renal parenchyma were significantly higher than ADChigh. ③Renal ADClow of control group, RAS, hydronephrosis, chronic nephropathy groups were (2.82±0.020)×10-3, (2.42±0.031)×10-3, (2.41±0.063)×10-3, and (2.31±0.037)×10-3 mm2/s, respectively. And ADChigh were (2.36±0.022)×10-3, (2.10±0.032)×10-3, (2.15±0.061)×10-3, and (1.98±0.043)×10-3 mm2/s, respectively. ADC values of three abnormal groups were significantly lower than that of normal control group. Conclusion Diffusion-weighted imaging may be a convenient method in evaluation of renal function and diseases. |
