目的 应用多普勒超声技术研究实验性兔VX2肿瘤性上腔静脉阻塞(SVCO)时,上腔静脉(SVC)的血流动力学变化,探讨多普勒血流频谱特征。方法 在超声引导下将VX2瘤组织悬液接种到SVC前壁前部的软组织内,建立SVCO动物模型。对14只实验兔于接种后12天,每隔2天经右锁骨上窝切面超声检测SVC血流变化,测量相关指标,并与接种前的超声测值比较。结果 随着肿瘤生长对SVC产生压迫,SVC受压和/或破坏,彩色多普勒显示SVC腔内逐渐出现五彩加速血流,至病变晚期血流信号反而减弱;频谱多普勒记录到病变早中期S波、D波血流速度分别为(78.25±14.97)cm/s、(59.68±13.16)cm/s,晚期各波速度分别为(33.71±15.90)cm/s、(33.55±20.03)cm/s。与晚期比较,在早中期SVC各波速度加快(P<0.001)。结论 兔SVCO时SVC彩色及频谱多普勒呈现典型狭窄图像,其多普勒图像特征与临床SVCO有较好的一致性。 |

Objective To study hemodynamic changes and Doppler imaging characteristics of superior vena cava obstruction (SVCO) in rabbit model of VX2 tumor with color Doppler ultrasound. Methods The VX2 tumor tissue suspension was injected into the soft tissues in front of anterior wall of superior vena cava (SVC) under ultrasound guidance. 14 rabbit models of SVCO were set up. The hemodynamic parameters of SVC in 14 rabbit models were observed and detected once every 3 days after 12 days post-injection. The flow parameters were compared with that of pre-inoculation. Results The wall of SVC was compressed and/or infiltrated by growing tumor. The mosaic flow on color Doppler was observed gradually in SVC as the obstruction was mild and moderate. The flow signal was weak when the obstruction was severe. At the early stage of the obstruction, the value of S and D wave of SVC spectrum were (78.25±14.97) cm/s and (59.68±13.16) cm/s respectively, while at the late stage was (33.71±15.90) cm/s and (33.55±20.03) cm/s. The velocities at early stage were higher significantly than that of late one (P<0.001). Conclusion The images of SVCO can be observed clearly in rabbit model by color Doppler ultrasound. The hemodynamic characteristics of the animal models of SVCO are concordance with that of human SVCO. |
