| 目的 探讨门静脉海绵样变 (CTPV) 的MRI表现,评价MRI 诊断CTPV 的临床价值及意义。方法 分析38 例经手术病理或DSA证实的CTPV 患者的MRI 平扫、动态增强及动态对比增强磁共振血管成像(DCE-MRA) 表现。结果 CTPV 的MRI平扫,所有病例门静脉主干或分支闭塞,周围可见由侧支静脉形成的团块状、网状异常软组织信号影;动态增强MRI,动脉期17例出现肝实质灌注异常,门静脉期全部病例见异常侧支静脉强化;DCE-MRA 能直观显示上述改变。结论 MRI 平扫、动态增强及DCE-MRA 检查对CTPV诊断具有重要价值,门静脉闭塞及其周围海绵样侧支静脉扩张为其特征性征象。 |

| Objective To investigate plain MRI scanning, dynamic enhanced and dynamic contrast-enhanced (DCE) MRA features of cavernous transformation of portal vein (CTPV), and to evaluate MRI in diagnosing CTPV. Methods Plain MRI , dynamic enhanced or DCE-MRA findings in 38 patients with CTPV confirmed pathologically or DSA proved were retrospectively analyzed. Results On plain MRI, obliteration of portal vein was found, and mass-like or reticular abnormal soft tissue signals were around the vein in all cases, which were produced by collateral vessels. On dynamic enhanced MRI scans, abnormal hepatic perfusion during arterial phase in 17 cases and abnormal enhanced collateral veins during portal phase were demonstrated in all cases. The above signs became more obvious on DCE-MRA. Conclusion Plain MRI, dynamic enhanced MRI and DCE-MRA are of great value for the diagnosis of CTPV, obliteration and collateral vessels around the portal vein is features of CTPV. |
