      目的 探讨多层螺旋CT容积显示技术对术前评价保留肾单位手术(NSS)的价值。方法 对25例拟行NSS的患者进行多层螺旋CT(MSCT)扫描及容积显示(VR)成像,观察肾脏位置、肿瘤的部位、范围及与肾动静脉、集合系统的关系,并与术中对照,9例同时与DSA检查进行比较。结果 MSCT及VR成像清楚显示了25例患者共32个肿块,与术中所见一致。VR成像显示肿块的部位、范围与术中所见一致。VR成像共显示了48条肾动脉中的45条(93.8%),显示了48条肾静脉中的42条(87.5%)。VR图像清楚显示了肾脏集合系统。结论 MSCT及VR成像能为外科医生提供拟行NSS较为准确、全面、便捷、无创的信息,对手术有积极的指导意义。
英文摘要:
      Objective To discuss the preoperative value of three-dimensional (3D) volume rendering multi-slice spiral CT(MSCT) for patients undergoing nephron sparing surgery (NSS). Methods MSCT and VR reconstruction were performed preoperatively on 25 consecutive patients undergoing nephron sparing surgery for renal neoplasms. The position of the kidney, the location and the extent of the tumor, the course of renal arteries and veins, and the relationship of the tumor to the collecting system were demonstrated. All CT findings were compared with the operative findings and DSA in nine patients. Results Before nephron sparing surgery, 32 renal tumors were identified in the 25 patients evaluated with MSCT and VR, which was confirmed correctly by operative findings. The location and extent of lesions identified by VR were accurate in all patients. Forty-five of 48 (93.8%) renal arteries and 42 of 48 (87.5%) renal veins were identified by VR. The collecting system was clearly demonstrated. Conclusion Three-dimensional volume rendering MSCT immediately provides the whole accurate information noninvasively to the surgeon, and plays the active roles in NSS.
