      目的 探讨窦汇区解剖形态的 MR血管成像(MRA)表现及其临床意义,用横窦血液流量MR测量方法对横窦的优势侧及非优势侧进行量化。方法 148例健康志愿者和体检者,用3D PC法进行窦汇区MR血管成像,对其中80例用2D PC法测量双侧横窦血流速度及血液流量。结果 ①横窦引流优势表现为右侧为主占54例,基本相等71例,左侧为主23例。②按照上矢状窦、直窦和双侧横窦的吻合关系,将窦汇区MR解剖形态分为三型;Ⅰ型,分三个亚型:Ⅰa型,上矢状窦和直窦汇合成一个真正的总池,从总池发出左右横窦。Ⅰb型,上矢状窦和直窦在终末端分成两股,左右分支汇合后构成同侧横窦。Ⅰc型,上矢状窦在末端发出两侧横窦,直窦汇入一侧横窦。Ⅱ型,分两个亚型:Ⅱa型,上矢状窦引流入右侧横窦,直窦引流入左侧横窦。 Ⅱb型,上矢状窦引流至左侧横窦,直窦引流到右侧横窦;Ⅲ型,分两个亚型:Ⅲa型,上矢状窦和直窦主要向右侧横窦引流。Ⅲb型,上矢状窦和直窦主要向左侧横窦引流。③右优势侧、双侧等势及左优势侧横窦的总血流量分别为(466.35±104.77) ml/min、(421.65±103.69) ml/min、(496.95±121.47) ml/min。结论 MRI是显示窦汇区解剖形态及脑静脉血流的有效方法,对窦汇区解剖形态的分类和横窦血液流量测量相结合,能为临床提供更多的信息。
英文摘要:
      Objective To investigate the anatomic appearance in the region of the torcular Herophili by using MR and evaluate its clinical significance, and to determine the venous drainage dominance from the point of quantity by measurement of the blood flow in transverse sinuses. Methods This study involved 148 healthy subjects. Images of venous sinuses were obtained in the region of the torcular Herophili with coronal 3D PC angiography, and 80 cases of these were examined to measure the blood flow velocity and the volumetric flow rates in transverse sinuses by 2D PC MR. Results ① The venous drainage dominance showed mainly to the right side in 54 cases, equal drainage in 71 cases, mainly to the left side in 23 cases. ② According to the relation among the sinuses in the region of the torcular Herophili, all subjects in this study could be grouped into 3 types; Type Ⅰ, including those subjects in which a confluence of sinuses existed or the superior sagittal sinus and the straight sinus forked, and the forking of both sinuses joined to form the lateral sinuses. Type Ⅱ, including those subjects in which the sagittal sinus drained into one side and the straight sinus drained into the other. Type Ⅲ, including those subjects in which the superior sagittal sinus and the straight sinus joined and mainly drained into the one side. ③ Total blood volumetric flow rates in trasverse sinuse of the right side drainage, equal drainage and left side drainge was (466.35±104.77) ml/min, (421.65±103.69) ml/min, (496.95±121.47) ml/min, respectively. Conclusion MR imaging provides a useful method to show the anatomic features in the region of the torcular Herphili and venuous blood flow of brain. A combination of the anatomic appearance in the region of the torcular Herphili with flow quantification of transverse sinuses can provide more useful information for clinicians.
