目的 探讨组织谐波显像对老年甲状腺局灶性病变的诊断价值。方法 对93例老年甲状腺结节患者分别进行常规超声检查和组织谐波显像检查,比较两种检查方法对老年甲状腺病变的显示能力。结果 93例患者共检出甲状腺结节232枚,结节大小(1.9±1.1) cm(范围0.3~3.6 cm)。基波显像171个结节界线清晰,58个结节界线模糊,3个结节界线难辨;而谐波显像199个结节界线清晰,32个结节界线模糊,1个结节界线难辨;谐波显像能够更好地显示结节的边界(P<0.001);谐波显像223个结节内部回声与甲状腺实质回声有明显差异,而基波现显像只有182个结节内部回声与甲状腺实质回声有明显差异,谐波显像能增强病灶组织回声的对比度(P<0.001)。对于病灶形态的显示,谐波和基波显像无明显差异(P=0.6321)。结论 谐波显像能增强老年人甲状腺结节边界的显示,增强病灶与周围组织的对比度,在老年甲状腺病变的发现、随访中有重要意义。 |

Objective To investigate the value of tissue harmonic imaging in diagnosis of thyroid lesions in elderly patients. Methods Ninty-three consecutive elderly patients with thyroid nodules were studied by both conventional ultrasound and tissue harmonic ultrasound. Two experienced ultrasonographer reviewed the images individually for the visibility of the nodule's margin and the relative gray scale contrast between lesions and adjacent thyroid tissue. The similarity of the lesions on sonogram in two vertical planes was also evaluated. Results A total of 232 thyroid nodules were found in 93 patients, and the mean diameter of the nodules was (1.9±1.1) cm, ranging from 0.3 to 3.6 cm. On conventional ultrasound margin of the lesions was well defined in 171 nodules and poorly defined in 58 nodules and ill defined in 3 nodules, while 199 nodules were well defined, 32 nodules poorly defined and 1 ill defined on tissue harmonic ultrasound. Tissue harmonic ultrasound demonstrated lesion's margin better than conventional ultrasound (P<0.001). Of all 232 lesions, tissue harmonic imaging showed obvious difference between lesions and adjacent thyroid tissue in 223 nodules, but only 182 nodues showed obvious difference on conventional ultrasound. Gray scale contrast between lesions and adjacent thyroid tissue was improved greatly on tissue harmonic ultrasound (P<0.001). Conclusion Tissue harmonic ultrasound can improve lesion's margin visualization and gray scale contrast between the lesions and the adjacent thyroid tissue, and may play important roles in the diagnosis of thyroid nodules in elderly patients. |
