      目的 分析良性与非典型、间变型脑膜瘤的18F-FDG PET影像,探讨定性诊断的可能性。方法 经术后病理证实为脑膜瘤25例,其中良性17例,非典型、间变型脑膜瘤8例,术前行脑18F-FDG PET显像。目视法观察各病灶的18F-FDG分布,计算各病灶的肿瘤/正常侧白质(T/WM)、肿瘤/正常侧灰质(T/GM)、肿瘤/小脑(T/Ce)的比值。结果 与对侧正常灰质比较,良性脑膜瘤病灶18F-FDG分布呈18F-FDG摄取减低,与对侧白质摄取相近似。非典型、间变型脑膜瘤病灶的18F-FDG分布,比对侧白质的摄取明显增高,与对侧正常灰质相近似。17例良性脑膜瘤的T/WM,T/GM,T/Ce的均数大小分别为0.91±0.14,0.31±0.17,0.58±0.23。8例非典型、间变型脑膜瘤的T/WM,T/GM,T/Ce的均数大小分别为1.54±0.18,1.07±0.21,1.14±0.31 (P<0.01)。结论 非典型、间变型脑膜瘤对18F-FDG的摄取明显高于良性脑膜瘤,18F-FDG PET显像可用于脑膜瘤定性诊断。

      Objective To analyze 18F-FDG PET images of patients with benign, atypical or anaplastic meningioma, and to explore the possibility of discrimination of them. Methods Twenty-five cases with meningioma were studied, 17 were benign and 8 were atypical or anaplastic meningioma. 18F-FDG PET scan was performed before operation. After getting images, distribution of 18F-FDG was visually viewed. Meanwhile, 18F-FDG uptake ratios of T/WM,T/GM,T/Ce were measured respectively. Results Compared with contralateral gray matter, there was a decrease of uptake of 18F-FDG in benign meningioma, but accumulation of 18F-FDG in atypical or anaplastic lesions was nearly equal to that in contralateral gray matter. Ratios of T/WM, T/GM, T/Ce in benign and atypical or anaplastic meningioma were 0.91±0.14, 0.31±0.17, 0.58±0.23 and 1.54±0.18, 1.07±0.21, 1.14±0.31, respectively. There was a significant difference in ratios of T/WM,T/GM,T/Ce between benign and malignant meningioma (P<0.01). Conclusion Uptake of 18F-FDG was higher in atypical or anaplastic meningioma than in benign ones. They could be differentiated with 18F-FDG PET through the change of accumulation of 18F-FDG.
