目的 评价MR平扫及动态增强扫描联合应用在MRCP诊断胰胆管疾病中的补充价值。方法 回顾分析64例胰胆管疾病病例的MRI表现,由两名有MR诊断经验的影像医生在无临床资料的情况下分为MRCP、MRCP与MR平扫结合、MRCP结合MR平扫及动态增强三步阅片,与手术、病理结果或临床综合诊断对照后进行对比分析。结果 第一、二步骤间定性诊断准确率、敏感度及特异度分别提高17.2%、14.7%及20%,差异显著(χ2=6.02,P<0.05),而第二、三步骤间差异不显著(χ2=1.74,P>0.05),但诊断准确率及确认度三步骤间均有显著差异。结论 MRCP应与MR平扫及动态增强扫描相结合,MR平扫与MRI动态增强扫描必要且有价值。 |

Objective To assess the supplementary value of unenhanced magnetic resonance (MR) images and dynamic enhanced images to MR cholangiopancreatography (MRCP) in diagnosis of pancreaticobiliary diseases. Methods MR findings of pancreaticobiliary diseases in 64 patients were reviewed. Two readers with experience in MRI diagnosis analyzed the images blindly by consensus in three steps (MRCP images alone, MRCP images with unenhanced MR images, and MRCP images with unenhanced and dynamic enhanced images). The Results were correlated with surgery, biopsy or clinic diagnosis, and then compared with each other. Results The discrepancy between MRCP images alone and MRCP images with unenhanced MR images in the accuracy in evaluating the nature of obstruction was significant (the sensitivity, specificity and accuracy were increased 17.2%,14.7% and 20% respectively, χ2=6.02, P<0.05), while the accuracy in evaluating the nature of obstruction not significant between MRCP images with unenhanced MR images and MRCP images with unenhanced and dynamic enhanced images(χ2=1.74,P>0.05). The discrepancies, however, among MRCP images alone, MRCP images with unenhanced MR images, and MRCP images with unenhanced and dynamic enhanced images were significant in the accuracy of diagnosis and the level of confidence. Conclusion MRCP should be associated with unenhanced MR images and dynamic MR gadolinium-enhanced images, which are necessary and valuable in diagnosis of pancreaticobiliary diseases. |
