Objective To investigate the change in global and regional ventricular function parameter measured by gated myocardial perfusion SPECT after surgery and to determine which quantitative parameter of WM and WT is more appropriate for the evaluation of regional cardiac. Methods Before and 3 to 5 week after CABG,40 patients (30 men,10 women) undertaken gated SPECT using 99Tcm-MIBI. Quantitative global and regional ventricular function analysis was performed using quantitative gated SPECT software. Results After CABG, global ejection fraction increased . End-diastolic and end-systolic volumes lessened significantly . As global function parameter, the changes in both total WM (r=0.75) and WT (r=0.79) correlated well with the change in ejection fraction after surgery. Segment analysis showed a significant postoperative increase in relative tracer uptake in the anterior, septal and inferior walls. Segment wall motion (WM) deteriorated in the septal,but increased in the lateral after CABG. However,WT did not decrease in septa areas and did not increase in lateral wall and correlated with percentage tracer uptake (r=0.73) better than WM did (r=0.24) in septal areas after CABG. Conclusion In patients with CABG postoperative WM analysis by gated myocardial perfusion SPECT underestimated septal motion and over estimated lateral motion. Therefore WTanalysis would be recommended for the evaluation of postoperative cardiac function. |
