目的 探讨弥散加权成像(diffusion weighted imaging, DWI)的rADC值测量在高级胶质瘤与单发脑转移瘤鉴别诊断中的应用价值。方法 对经手术病理证实的27例高级胶质瘤与19例脑转移瘤,在术前接受了常规MRI检查及DWI。分别测定感兴趣区ROI的ADC值,并计算其rADC值。结果 高级胶质瘤瘤体的rADC值(1.27±0.17)与单发脑转移瘤瘤体的rADC值(1.23±0.18)间无显著性差异(P>0.10);高级胶质瘤远瘤周区的rADC值(2.37±0.34)与单发脑转移瘤上述区域的rADC值(2.48±0.44)间亦无显著性差异(P>0.10);但高级胶质瘤近瘤周区的rADC值(1.41±0.21)与单发脑转移瘤上述区域的rADC值(2.25±0.17)间存在显著的差异(P<0.001)。结论 测定高级胶质瘤与单发脑转移瘤瘤体及远瘤周区的rADC值在二者鉴别诊断中的价值不大,测定二者近瘤周区的rADC值在二者鉴别诊断中具有一定的价值。 |

Objective To explore the value of relative apparent diffusion coefficient (rADC) values of the peritumoral region in differentiating high-grade gliomas from solitary metastases. Methods Before operation, 46 patients with brain tumour were examined by conventional MRI and DWI, in which 27 cases were proved to be high-grade gliomas and 19 cases be solitary metastases by histopathology. The ADC values of ROI in different part of high-grade gliomas and solitary metastases, including tumor, adjacent and far peritumoral region were analyzed and the rADC values were calculated. Results There was no significant differences in rADC value between high-grade gliomas (1.27±0.17), (2.37±0.34) and solitary metastases (1.23±0.18), (2.48±0.44) in tumor and far peritumoral region. However, a significant difference was found in rADC value of high-grade gliomas (1.41±0.21) as compared with that of solitary metastases (2.25±0.17) in the adjacent peritumoral region (P<0.001). Conclusion The rADC in tumor and far peritumoral region has no significance in distinguishing high-grade gliomas and solitary metastases, whereas the rADC in adjacent peritumoral region plays an important role in distinguishing them. |
