      目的 从病理角度探讨功能性肺叶切除的可行性。方法 经导管靶肺叶内注入平阳霉素碘油乳化剂及聚甲基丙烯酸甲脂支气管阻塞建立犬功能性肺叶切除的模型,于术后不同时间处死动物,按Szapiel方法测定肺泡炎和肺纤维化的程度,观察肺组织的病理变化。结果 模型组靶肺第1~7天以肺泡炎为主,第3天开始出现肺纤维化改变,第14天以后则进入慢性纤维化期,第28天完全纤维化。结论 功能性肺叶切除可使靶肺组织不张、纤维化,作为治疗肺部病变的一种微创、安全、有效的介入治疗方法,可望部分替代外科性肺叶切除。
英文摘要:
      Objective To investigate the possibilities of functional pulmonary lobectomy (FPLT) by pathology. Methods To establish the canine model of FPLT, Pingyangmycin (PYM) lipiodol emulsion was injected to the target-lung, and the target-bronchia was obstructed with Polymethylmethacrylate (PMMA) by catheter. Animals were sacrificed on different time,and the lungs were harvested for pathological examination.The degree of pulmonary fibrosis and alveolus inflammation were measured by Szapiel method.Results The target-lung lesions of experimental group were mainly alveolitis on days 1-7 and pulmonary fibrosis was appeared on day 3, and advanced to the chronic stage of pulmonary fibrosis after day 14, on day 28 pulmonary fibrosis was increased significantly. Conclusion Pulmonary fibrosis can be obtained by FPLT, and FPLT is a mini-invasive, safe and effective method and might be partially replace the surgical pulmonary lobectomy in future.
