      目的提出一种基于三维体积测量软件的半自动分段方案,使海马结构MRI测量成为一种临床上实用的诊断方法。方法 对31例正常受试者行矢状位及直接斜冠状位容积采集。应用组内相关系数(ICC)及一致性相关系数(rc)评价三维分段方案的信度及标准效度。并分析影响海马体积的因素。结果 半球侧别、性别及颅内体积均与海马原始绝对体积有关。经协方差法及相除法校正后,性别及颅内体积的影响消失或减小。以协方差方法校正效果为佳。结论 基于三维软件的海马结构半自动分段方案具有高度的可重复性、准确性及真实性,并可以明显提高海马结构分段速度。

      Objective To establish a semiautomated volumetric method of HF based on three-dimensional software program with excellent accuracy, reliability and high speed of segamentation, and to make the HF volumetry on MR images a practical clinical method. Methods The T1 volumes were acquired using a three-dimensional SPGR with sagittal and tilted coronal volume excitation (perpendicular to the axis of the right hippocampal formation) in 31 normal volunteers. Reliability and standard validity were calculated using intraclass correlation coefficients (ICC) and concordance correlation coefficients (rc) respectively. The effect factors on HF volumes, normalization of HF volumes and asymmetry of right and left HF in volunteers were assessed.Results The mean native volume of the right (3.843±0.464 cm3) was greater than that of the left (3.750±0.452 cm3) (P<0.05). The intrarater ICC for right and left hippocampus was 0.947 and 0.956 respectively, the interrater ICC for right and left hippocampus was 0.886 and 0.853 separatively (P<0.05). The rc of right hippocampus was 0.964 and that of left hippocampus was 0.978. Those results suggested that the reliability and validity of the volumetric method established by the authors were excellent. The hemispheres of HF, sex and intracranial volumes of subjects all had relationship with native absolute volume of HF. A significant difference between the male and female hippocampal volumes (P<0.05), with the men having larger hippocampus. The subjects who had larger intracranial volumes had larger hippocapmal volume (P<0.01). The correlations between the age of volunteers and hippocampus volume were not significant (P>0.01). The mean intracranial volumes of male were large than those of female (P<0.05). After normalization by division of hippocampal volume by intracrainal and the covariance method, the sex and intracranial effect on hippocampal volume reduced or eliminated. Correction via the covariance method was superior to the correction via division. The absolute mean volume of right and left HF after correction via the covariance method were (3.843±0.258) cm3 and (3.750±0.240) cm3, respectively. The relative volume of HF(δV) was -2.427±3.280. All the relative volumes of HF fell within two standard deviations from the mean of the values for the control subjects and all the absolute volumes of HF were below the normal range.Conclusion The volumetric method of HF provided by the authors has high reliability, accuracy and validity. The speed of volumetry can be increased obviously.
