目的通过动物模型研究缺氧缺血后急性期脑组织表观扩散系数变化的病理基础。方法 选用出生后3~7天的健康新生猪25头,分正常对照组5头和2个模型实验组,测定额顶区、基底节区、海马区的ADC值变化,并观察组织学改变。结果 单侧颈总动脉永久性结扎模型组的ADC下降程度轻,48 h后开始部分恢复;组织学上胶质细胞可见轻度水肿,神经元细胞的改变不明显。双侧颈总动脉暂时性结扎同时乏氧模型组的ADC变化呈持续下降趋势,在72 h内未见恢复;组织学上见典型的皮层神经细胞板层样病变,严重者见皮层下局灶性梗死,神经、胶质细胞凋亡明显。结论 ADC值变化与神经、胶质细胞水肿、凋亡及由此产生的水分子无序运动有关。 |

Objective To explore the histopathological correlations with the apparent diffusion coefficient (ADC) changes in neonatal piglets of hyperacute cerebral hypoxia /ischemia (HI). Methods Twenty-five piglets at the age of 3 to 7 days were divided into one control group and two experiment groups, and ADCs value was measured in frontoparietal region, basal ganglia and hippocampus. Results ADC reduction in Model 1 was mild and recovered partialy after 48 h, in which histological examination showed mild glial cells swollen. Persistent decline of ADC was seen in Model 2 without recovery within 72 h. Histological examination showed cortical neurons with typical laminar necrosis. Severe cases showed focal infarction, in which neurons and glial cells apoptosis could be seen obviously. Conclusion ADC changes are correlated with neurons and glial cells swollen, apoptosis and the out-of order motion of water molecules. |
