      目的 探讨单侧ICA/MCA狭窄与闭塞患者MR脑灌注的MTT和TTP表现。方法 90例患者(ICA 62例、MCA 28例)行MR脑灌注检查,比较MTT和TTP灌注延迟体积,分析灌注延迟表现与病变动脉及其狭窄程度关系。结果 90例患者MTT和TTP灌注延迟体积无显著性差异,延迟表现分三型:Ⅰ型仅累及MCA区,Ⅱ型仅累及分水岭区,Ⅲ型累及MCA和分水岭区。ICA患者Ⅲ型40例(65%), MCA患者Ⅰ型24例(86%)。结论 MTT、TTP能够敏感显示灌注损伤,对ICA/MCA重度狭窄或闭塞诊断、治疗及脑梗死发病机制研究有重要价值。

      Objective To probe the application of mean transit time (MTT) and time-to-peak (TTP) parameters of MR cerebral perfusion-weighed imaging (PWI) in patients with unilateral ICA/MCA high-grade stenosis or occlusion. Methods PWI was performed on 90 patients with ICA (62)/MCA (28) high-grade stenosis or occlusion. The extents of perfusion delay on MTT and TTP were compared, and the findings were analyzed. Results There was no statistically significant difference between the extents of abnormal on MTT and TTP in these 90 patients. Three types of MR findings were found, including Ⅰ: involving MCA territory, Ⅱ: involving border zone, and Ⅲ: involving MCA territory and border zone. Forty (65%) were type Ⅲ in 62 patients with ICA stenosis or occlusion, while 24 (86%) were type Ⅰ in 28 patients with MCA. Conclusion MTT and TTP were sensitive to depict abnormal perfusion, and thus play an important role in diagnosis,treatment and study pathogenesis of ICA/MCA stenosis or occlusion.
