      目的 评估计算机辅助检测(CAD)对乳腺良、恶性肿块及钙化灶检出的稳定性。方法 对经病理证实的207例乳腺肿块患者乳腺钼靶X线片行三次CAD扫描,与影像诊断及病理结果进行对照。结果 三次CAD扫描在肿块及钙化灶检出部位上存在差异。CAD检出Ⅲ级的恶性及良性病灶分别为75.7%和58.2%;对良、恶性钙化灶的稳定性分别为46.1%和67.2%;CAD一次扫描与三次扫描的真阳性率无显著性差异(χ2=0.96,P>0.05)。结论 CAD对恶性肿块检出稳定性较良性病变为高;对恶性钙化灶检出敏感性较高,但稳定性较低。
英文摘要:
      Objective To evaluate the reproducibility of computer-aided detection (CAD) in diagnosing benign and malignant breast lesions. Methods Total 207 patients with breast lesions were proved pathologically after surgery. All mammograms of these patients were scanned and analyzed three times by the CAD system. Reproducibility of detection sensitivity and the individual CAD-generated cues were compared with pathological findings. Results There were significant differences in the detected regions of masses and microcalcification among the three scans. 75.7% of malignant mass and 58.2% of benign mass were marked on all three images. As to microcalcification, 46.1% of benign regions was marked, 67.2% of malignant regions was marked. There was no significant (χ2=0.96,P>0.05) differences in true-positive rate between one scan and there scans. Conclusion The reproducibility of CAD to the malignant regions was higher than that of benign regions. There was no significant difference in true-positive rate between times of scan. The sensitivity of microcalcification regions was higher than that of mass regions, but the reproducibility of detection sensitivity was lower.
