| 目的 探讨窦口鼻道复合体病变及解剖变异与慢性上颌窦炎的关系。方法 对CT诊断的52例单侧慢性上颌窦炎进行两侧对比分析。结果 患侧与健侧筛泡截面分别为(77.75±33.51) mm2和(34±20.27) mm2(P<0.01)。两侧分别有19个及10个大筛泡。患侧伴有32组筛窦炎,对侧21组。鼻息肉11例,单发于上颌窦炎侧9例,对侧仅1例,双侧1例。结论 大筛泡、前中组筛窦炎症、鼻息肉与慢性上颌窦炎有关。 |

| Objective To evaluate the relationship of lesions and anatomic variation of ostiomeatal complex (OMC) with chronic maxillary sinusitis. Methods Total 52 cases with unilateral chronic maxillary sinusitis were examined by CT. The CT findings of bilateral maxillary sinus were compared. Results The cross-section area of ethmoid bullae with maxillary sinusitis was (77.75±33.51) mm2, whereas the other was (34±20.27) mm2(P<0.01).There were 29 large ethmoid bullae, in which 19 located in the same side with maxillary sinusitis and other 10 located in the opposite side (P<0.05). The anterior and middle ethmoid sinusitis was found in 35 cases, in which 32 sinuses with ethmoid sinusitis located in the same side with maxillary sinusitis and 21 in the opposite side (P<0.05). There were 11 patients with nasal polyp, 1 in both sides, 9 in the same side with maxillary sinusitis, another in the opposite side (P<0.05). There was no statistical difference of deviation of nasal septum, Haller's cells and pneumatized middle turbinate in both sides. Conclusion Local lesions and anatomic variation are possible factors for causing chronic maxillary sinusitis, which include large ethmoid bulla, anterior and middle ethmoid sinusitis and nasal polyp. The cross-sectional area of large ethmoid bulla and volume of nasal polyp is significantly related to the occurrence of maxillary sinusitis. There is no significant relationship of deviation of nasal septum, Haller's cells and pneumatized middle turbinate with chronic maxillary sinusitis. |
