      目的 探讨多层螺旋CT对环杓关节脱位的诊断价值。方法 28例单侧声带麻痹患者,在平静呼吸及Valsalva呼吸状态下,分别行喉部多层螺旋CT扫描及容积重建,治疗后复查CT。结果 杓状软骨前脱位16例,后脱位12例。声带固定于旁中位伴同侧梨状窝、喉室扩大26例,固定于中位伴梨状窝缩小2例,复位后18例环杓关节显示正常。结论 多层螺旋CT能清晰显示环杓关节脱位的病变特征,对环杓关节脱位诊断与复位治疗具有重要的临床价值。

      Objective To evaluate the value of multi slice spiral CT (MSCT) in diagnosing arytenoid dislocation. Methods Twenty eight patients with hoarseness and lateral vocal cord paralysis were examined with MSCT. Twice scans were performed during quiet breath and Valsalva breath respectively, and volume rendering (VR) images were gained on the workstation. Results Anterior and post arytenoid dislocation were in 16 cases and 12 cases, respectively. The paralyzed vocal cord fixed at paramedian position accompanying with enlargement of the ipsilateral pyriform sinus and ventricle were in 26 cases, while 2 cases with diminution of the ipsilateral pyriform sinus and ventricle. Eighteen cases of the improved position of the arytenoid were confirmed by a subsequent CT scan. Conclusion MSCT can clearly demonstrate the features of arytenoid dislocation and may be useful in the diagnosis and evaluating the repositioning of arytenoid dislocation.
