目的 应用MRI定量研究尿道前列腺部长度(UL)、前列腺总体积(PV)、中央带体积(CV)和中央带体积指数(CVI),探讨前列腺形态学参数与男性下尿路梗阻程度间的相关性。方法 62例除外神经、内分泌系统病史和尿路外伤史的男性下尿路梗阻患者行前列腺MRI和尿流动力学检查。根据最大尿流率(MFR)进行分组,即正常组(Ⅰ)、可疑梗阻组(Ⅱ)、梗阻组(Ⅲ)三组。盲法观察、测量和计算前列腺形态学参数,对患者的年龄、前列腺形态学参数及尿流动力学参数进行统计学分析。结果 本组研究对象的UL、PV、CV、CVI与年龄无明显相关,MFR和年龄呈中等程度负线性相关,相关系数r=-0.409, P=0.001;三组患者的年龄、PV无显著性差异;UL、CV、CVI在Ⅰ组和Ⅲ组间存在显著差异,而在Ⅰ组和Ⅱ组、Ⅱ组和Ⅲ组间则无显著性差异; UL、CVI与梗阻程度相关,偏相关系数B分别为0.178和1.025,t值分别为2.325和2.131,P值均<0.05。结论 UL和CVI在正常组和梗阻组之间存在显著性差异,两者可作为预测下尿路梗阻程度的相关形态学参数。 |

Objective To determine if prostate urethra length (UL), total prostate volume (PV), central zone volume(CV) or central zone volume index (CVI) measured by MRI are correlated with the severity of male lower urinary obstruction. MethodsMRI and urodynamic examination were carried out in 62 men respectively excluded the urethral trauma and the diseases of nerve system and endocrine system. Using the maximum flow rate (MFR) as criteria, the patients were divided into three groups: normal, suspected lower urinary obstruction and lower urinary obstruction. UL and three dimensional length of whole prostate and central zone were measured respectively by the observer blinded of the urodynamic results. PV, CV and CVI were calculated. ResultsA moderate negative pairwise correlation was observed between MFR versus age with r=-0.409 and P=0.001 in all groups. No line correlation between age versus UL, PV, CV and CVI were observed in all groups. The difference of age and PV in all groups is not statistically significant, whereas the difference of UL, CV and CVI between the normal and lower urinary obstruction group was statistically significant. UL and CVI were considered as correlative factors with the severity of lower urinary obstruction with B value of 0.178 and 1.025 respectively. t value was 2.325 and 2.131 respectively. Both P value was less than 0.05. Conclusion The differences of UL and CVI between the normal and lower urinary obstruction group are statistically significant. UL and CVI are the useful morphological parameters for predicting the severity of the lower urinary obstruction. |
