| 目的 同层动态磁共振胰胆管造影(MRCP)评价Oddi括约肌功能。方法 15例正常人、38例患者(分成Ⅰ、Ⅱ、Ⅲ组),分别行常规及同层动态MRCP。结果 在括约肌段舒张人数及平均舒张次数方面,Ⅰ组与其他各组间有明显差异(P<0.05);胆总管远端倒杯口状仅出现于Ⅰ组。正常组及Ⅱ、Ⅲ组中,动态MRCP能观察到括约肌段舒张的人数明显多于常规MRCP(P<0.05);对于Ⅰ组两种方法虽无明显差异,但动态MRCP诊断可信度更高。结论 同层动态MRCP明显优于常规MRCP,可减少对壶腹部病变诊断的假阳性,提高诊断的准确性及可信度。 |

| Objective To evaluate the function of Oddi's sphincter with dynamic MRCP of the same slice. Methods Fifteen healthy volunteers and 38 patients (Group Ⅰ, Ⅱ, Ⅲ) underwent conventional and dynamic MRCP, and the image data were analyzed respectively. Results There were significant differences between Group Ⅰ and other groups at the number of persons who had relaxations of the sphincteric segment and at the average number of relaxations of each person on dynamic MRCP images (P<0.05). The meniscoid shape of the distal end of biliary duct appeared only in Group Ⅰ. There were more persons who had relaxations on dynamic MRCP than those on the conventional MRCP in volunteers and Group Ⅱ, Ⅲ(P<0.05), and there was no significant difference in Group Ⅰ for the two imaging techniques, but the confidence in diagnosis was higher for dynamic MRCP than that for conventional MRCP. Conclusion Dynamic MRCP is helpful in the diagnosis and differential diagnosis of ampullary and periampullary obstructive lesions, and is obviously better than conventional MRCP. Clinical application of dynamic MRCP will decrease the false positivity rate and increase the diagnostic accuracy. |
