      目的 探讨肝细胞癌(HCC)螺旋CT表现特征与血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)、血管生成及其他因素之间的关系。方法 回顾性分析50例(54个)经CT双期增强扫描的HCC病灶,将CT表现特征与免疫组化、临床病理结果进行对比分析。结果 Logistic回归分析显示:与动脉期强化类型有关的因素是病灶大小和瘤内坏死;与包膜类型有关的因素是FⅧRA表达部位和微血管密度(MVD);与侵袭转移性有关的是VEGF和MVD;与病灶大小有关的是AFP水平;与肝硬化有关的是HBsAg。结论 HCC的CT表现特征受HCC血管生成、VEGF表达等多个因素的影响。
英文摘要:
      Objective To investigate the relationship between the contrast enhancement features on spiral computed tomography and the expressions of vascular endothelial growth factor (VEGF), basic fibroblast growth factor (bFGF), angiogenesis and some clinicopathological characteristics of hepatocellular carcinoma (HCC). Methods Fifty cases (54 lesions) of HCC examined with enhancement dual-phase CT scanning (arterial phase and portal vein phase) were analyzed retrospectively. The CT features were compared with the immunohistochemical and some clinicopathological characteristics. Results The enhancement patterns were related with the diameter of tumors and intratumoral necrosis, and the capsulae patterns were associated with the expression distributions of FⅧRA and microvessel density (MVD) with logistic regression analysis. There was significant correlation between the high infiltration and severity (intrahepatic daughter foci, tumor-emboli in portal veins, lymphatic metastasis) on the CT images and VEGF or MVD. The same correlation was found between the diameter of tumor on the CT images and AFP level, and also between hepatic cirrhosis on the SCT images and HBsAg. No significance was found among bFGF, the microvessel diameter and CT features. Conclusion The CT features of HCC are influenced by many factors including angiogenesis, the expression of VEGF and other factors together.
