| 目的 探讨多层CTA三维重建(3D-MSCTA)与脑血管造影数字减影术(IADSA)诊断脑动脉瘤的临床价值。方法 对37例自发性蛛网膜下腔出血患者行3D-MSCTA及IADSA检查。以手术或随访为标准评价两种检查结果及图像质量。结果 发现脑动脉瘤29例34处。两种检查的敏感度、阳性预测值、阴性预测值间有极显著性差异(P<0.01),特异度、准确度间及VR/MIP、IADSA敏感度和阴性预测值间有显著性统计学差异(P<0.05)。图像质量评估数值最高为VR,组间有显著性差异(P<0.01)。结论 对于脑动脉瘤的诊断,3D-MSCTA比DSA更敏感、更准确。 |

| Objective To evaluate the clinical value of three-dimensional multislice CTA and digital subtraction angiography for diagnosis of cerebral aneurysms. Methods Thirty-seven patients who had subarachnoid hemorrhage underwent 3D-MSCTA and IADSA examinations. All results of 3D-MSCTA and IADSA examinations and image quality of three reconstruction techniques of 3D-MSCTA and IADSA were assessed. Results Twenty-nine patients had 34 cerebral aneurysms. There was significant difference in sensitivity,positive predictive value and negative predictive value between 3D-MSCTA and IADSA (P<0.01). Besides, there was statistical difference between specificity and accuracy (P<0.05). Statistical difference was also found in sensitivity and negative predictive value between VR/MIP and IADSA (P<0.5). For the score of image quality, VR was the highest, markedly significant differences were demonstrated between the two techniques (P<0.01). Conclusion 3D-MSCTA was proved to be more sensitive and accurate than IADSA in evaluating cerebral aneurysms. |
