目的 应用磁共振相位对比法,观察Chiari Ⅰ畸形合并脊髓空洞病人脑脊液的流动情况,并进行定量分析。方法 对14例Chiari Ⅰ畸形合并脊髓空洞患者及14例正常成人进行矢状位扫描,在正中矢状位上测量延前池及C2-3水平蛛网膜下腔腹背侧宽径;并对其中10例患者及10例正常成人应用磁共振相位对比序列,配合以回顾性心电门控技术,分别测量中脑导水管、枕大孔区、C2-3间盘水平各层面脑脊液流速。结果 与对照组比较,合并空洞的Chiari Ⅰ畸形患者延前池及颈椎蛛网膜下腔均变窄(P<0.05);中脑导水管足向CSF最大流速增大(P<0.05),头向CSF最大流速明显增大(P<0.01);枕大孔区足向及头向CSF最大流速较对照组明显增大(P<0.01);C2-3间盘水平足向CSF最大流速明显增大(P<0.01),头向CSF最大流速与对照组比较无明显差异(P>0.05);患者的R-R间期较对照组缩短(P<0.05)。结论 Chiari Ⅰ畸形合并脊髓空洞患者的脑脊液流动增强,推测对空洞的形成可能起重要的作用。 |

Objective To quantitatively study the features of cerebrospinal fluid flow dynamics in Chiari Ⅰ malformation with syrinx by using phase-contrast MRI. Methods Fourteen patients and fourteen healthy subjects underwent sagittal MR scan. To measure lengths of prebulbar cistern and ventral and dorsal spinal subarachnoid spaces at the C2-3 level. Among these subjects,ten patients and ten healthy subjects underwent retrospectively gated phase-contrast MRI scan. Velocities were measured at three regions of interest:aqueduct, ventral foramen magnum and ventral spinal subarachnoid spaces at the C2-3 level. Results The lengths in patients were significantly decreased compared with these in healthy subjects (P<0.05). The increased systolic velocity (P<0.05) and diastolic velocity (P<0.01) of aqueduct were identified;the systolic velocity and diastolic velocity of ventral foramen magnum increased distinctly (P<0.01);the systolic velocity of C2-3 level increased significantly (P<0.01),while the diastolic velocity was unaltered. Decreased R to R interval in patients was also demonstrated (P<0.05). Conclusion Cerebrospinal fluid flow was enhanced in Chiari Ⅰ malformation with syrinx,which was supposed to facilitate the development of syrinx. |
