      目的 研究糖尿病患者视网膜中央动脉(central retinal artery,CRA)与眼动脉(ophthalmic artery,OA)的血流动力学改变及其与血糖、病程之间的关系。方法 利用彩色多普勒超声诊断仪检测55例(110只眼)正常人和46例 (92只眼)糖尿病患者的CRA及OA。结果 ①糖尿病患者(有或无糖尿病视网膜病变)CRA的收缩峰值血流速度(PSV)、舒张末期血流速度(EDV)、平均血流速度(Vm)显著降低,CRA及OA的阻力指数(RI)显著升高。②糖化血红蛋白与CRA、OA的EDV呈显著负相关,与RI呈显著正相关。③病程与CRA的血流速度、OA的EDV呈显著负相关,与OA的RI呈显著正相关。结论 糖尿病患者的CRA、OA血流速度降低,视网膜及脉络膜微循环阻力均增加;而且血糖、病程是重要的影响因素。

      Objective To observe hemodynamic changes of central retinal artery (CRA) and ophthalmic artery (OA) and their relations with blood glucose and duration in diabetic patients. Methods Hemodynamic parameters of CRA and OA were investigated in 55 healthy volunteers (110 eyes) and 46 diabetic patients (92 eyes) by color Doppler flow imaging (CDFI). Results ①Comparing with healthy group, blood flow velocity parameters of CRA decreased significantly and the resistance index (RI) of CRA and OA increased significantly in diabetic patients (without or with diabetic retinopathy). ②There was a negative correlation between EDV of CRA, OA and glycosylated hemoglobin (HbA1c),a positive correlation between RI and HbA1c. ③The blood flow velocity parameters of CRA and EDV of OA were correlated with diabetic duration negatively, but RI of OA was correlated with duration positively. Conclusion In diabetic patients, the blood flow velocity of CRA and OA decreased, the resistance of retinal and choroid microcirculation increased. Blood glucose and duration were the main factors that influenced the ocular hemodynamics.
