目的 应用心肌速度梯度对肥厚型心肌病左室短轴方向局部收缩功能进行评价。方法 应用定量组织速度成像技术分析16例HCM患者和15例正常人室间隔左室面、右室面下与左室后壁内、外膜面下心肌短轴方向的速度曲线,测量收缩期峰值运动速度(Vs)、收缩期心肌速度梯度(MVGs)等参数。结果 HCM组室间隔左、右室面下与左室后壁内、外膜面下心肌短轴方向Vs与正常人相比较无显著性差异,但其室间隔与左室后壁短轴方向MVGs明显低于正常对照组;室间隔收缩期峰值速度梯度与IVS/PW之间显著相关,而左室射血分数与IVS/PW比值之间相关性差。结论 HCM左室短轴方向局部收缩功能是降低的;心肌速度梯度能够精确反应室壁局部收缩功能,定量组织速度成像为我们提供了一种测量心肌速度梯度的新方法。 |

Objective This study was performed to assess the regional left ventricular systolic function from short-axis view by myocardial velocity gradient in patients with hypertrophic cardiomyopathy. Methods In 16 patients with hypertrophic cardiomyopathy and 15 normal subjects, the subendocardial and subepicardial motion velocity patterns at the middle of the left ventricular posterior wall (LVPW) and interventricular septum (IVS) in left ventricular parasternal long-axis view were analyzed by QTVI. Systolic peak velocities (MVGs) on subendocardium and subepicardium of IVS and LVPW,systolic myocardial velocity gradient between endocardium and epicardium were measured. Results In term of subendocardial and subepicardial peak systolic velocities of IVS and LVPW, there was no statistically significant difference between HCM and normal groups. Compared with control group, the systolic MVG of IVS in HCM group was significantly lower (P<0.001), so was the systolic MVG of LVPW. There were significant correlations between systolic MVG of IVS and the IVS to LVPW thickness ratio (r<sub>mvg=0.623). On the contrary, the correlations between LVEF and the IVS to LVPW thickness ratio were poor (r<sub>EF=0.189).Conclusion The results in this study demonstrated that regional left ventricular systolic function was abnomal in HCM, and the extent of this abnormality was related to the degree of asymmetry. Myocardial velocity gradient has potential for the assessment of regional systolic function accurately, and QTVI could offer a new approach to calculate myocardial velocity gradient. |
