      目的 定量分析强迫症(OCD)患者静息和症状诱发状态下不同脑区的血流动力学变化。方法 10例OCD患者两种状态下行MR灌注成像(PWI),测量不同脑区的相对脑血流量(rrCBF),并进行诱发前后的对比。结果 症状诱发后,AAS与OCDAS均有增加(P<0.01);左侧额下回、前扣带回,右侧尾状核头、壳核,双侧眶额皮质、丘脑及杏仁核的局部相对脑血流量(rrCBF)增加(P<0.05);左侧额下回、右侧壳核的rrCBF与OCDAS呈负相关 (相关系数分别为 -0.614、-0.562, P值分别为0.029、0.045)。结论 PWI能定量分析OCD症状诱发前后脑血流动力学改变。

      Objective To quantitatively study the changes of hemodynamics of obsessive-compulsive disorder (OCD) cases in resting and symptomatic states. Methods MR perfusion-weighted images (PWI) were performed during both resting and symptom provocation states in OCD cases, and the change of regional relative cerebral blood flow (rrCBF) was measured and compared. Results The rrCBF was significant different between the symptomatic state and the resting state within several cortices and subcortical structures (including the left inferiofrontal cortex, anterior cingulate cortex; the right head of caudate nucleus, putamen; and the bilateral orbitofrontal cortex, thalamus, and amygdala) (P<0.05). For the provoked state, the rrCBF within the left inferiofrontal cortex and the right putamen were correlated negatively with the OCD analogue scale (OCDAS) scores (the correlation coefficients were -0.614,-0.562, respectively; and the P values were 0.029, 0.045 respectively). Conclusion PWI can provide quantitative information of the cerebral hemodynamics with symptom provocation in patients with OCD.
