      目的 探讨肝肿瘤介入治疗后超声随访的内容、方法和价值。方法 对超声引导微波治疗、超声引导酒精治疗和经放射介入治疗的56例肝肿瘤病人进行超声随访和疗效分析。包括:肿块的二维大小、三维体积;肿块的彩色血流多少、分布情况、三维血管树容积;肿块的内部回声、强弱变化。与同期检查的CT/MRI、临床放射免疫指标和/或手术病理进行对比分析。结果 肿块内血流分级可以反映肿瘤血供情况,三维体积测定与实际肿块体积误差<15%。结论 超声疗效分析可以作为肝肿瘤介入治疗后疗效及预后判断的有效方法。肿瘤内血流的检测是随访的重要内容,介入治疗后血供的增减情况可以作为是否手术的参考指标;三维容积测定更能反映瘤体大小的变化。

      Objective To discuss the value and methods of ultrasonography in following up the liver tumor after interventional treatment. Methods Follow up ultrasonography was performed on 56 patients with liver tumor treated by microwave, ultrasound guided alcohol inspiration or radio-intervention. Follow up ultrasonography included: the size of the mass measured by two-dimensions, three-dimensions (3D), the quantity, ditribution of the vascularity in the mass, the volume of the 3D vessel-tree and the changes of echogenicity in the tumor. The effects of the therapy were analyzed. The results of follow-up ultrasonography were compared with CT, MRI, radioimmunology and/or the pathology. Results The grade of the vascularity of the tumor is related to blood supply in the tumor. The difference in measuring the size of the tumor between 3D volume and gross volume of pathology is less than 15%. Conclusion The ultrasound is an effective method to assess the potency and the prognosis of the tumor in the liver after interventional treatment. Assessing vascularity of the tumor is an important part in routine follow-up ultrasonography. The grade of vascularity in the tumor can be an important indication for choosing either resection of the tumor or non-surgical treatment. 3D volume is more accurate than 2D measurement in evaluating the size of the tumor in the liver.
