      目的 评价磁共振2D-FIESTA序列能否用于婴儿肝移植术后肝外胆道及血管系统的显示。方法 3例婴儿肝移植患者术后行6次MR检查以了解肝外胆道及血管的通畅情况。患儿口服水合氯醛或静注安定镇静。行轴位、冠状和矢状2D-FIESTA扫描。不需屏气。不使用MR造影剂。在ADW工作站上以MIP和MPR方式重建图像。结果 2D-FIESTA序列可清晰显示患婴的肝外门静脉、肝静脉、下腔静脉及肝外胆管,肝动脉显示欠清晰。1例显示来自供体的门静脉过长,1例显示下腔静脉吻合口狭窄。1例显示多发肝脓肿和膈下积液。结论 2D-FIESTA序列可在自由呼吸、不使用造影剂的条件下显示婴儿肝移植患者的肝外静脉血管和肝外胆管。

      Objective To evaluate whether the hepatic vessels and biliary tract could be visualized with a rapid non breath-hold technique (2D-FIESTA) in the infant recipients of liver transplantation. Methods Three infant patients underwent liver transplantation using bipartition procedure. The MR examinations were performed in March 2003. Images were obtained with a 1.5T whole body MRI system by using the CARDIAC coil with high performance gradients. The patients were sedated by chloral hydrate orally. No contrast agents were required. The sequence was performed by means of axial, coronal and sagittal non breath-hold, non respiratory-gated, fat suppressed, two-dimensional, heavily-T2 weighted FIESTA sequence. The imaging parameters of which were: TR=4.1 ms; TE=1.6 ms; section thickness 2.6-3 mm with no interslice gap; field of view 22 cm; matrix size 256×256 pixels; 1 signal averaged. The receiver bandwidth was 125.0 kHz, and sequential and multiphase options were used. The images were acquired over a period of 37 to 50 seconds. All of the axial, coronal and sagittal images were reviewed on an independent workstation and maximum intensity projections and multiplanar reformatting techniques were applied. Results The 2D-FIESTA pulse sequence results in images in which the recipient hepatic vascular structures and the biliary tract were highlighted. In this way, a clear visualization of the hepatic vessels and biliary tract could be obtained in all 3 cases. In one case it was demonstrated that a redundant portal vein left from the donator liver. In one case it was displayed that a inferior vena cava stricture at the anastomosis site. And multiple hepatic abscesses and subhepatic fluid collection were also detected. Limitation of 2D-FIESTA sequence revealed by our study was the mild motion artifact of the aorta and the hepatic artery in the axial images. Conclusion 2D-FIESTA is a fast sequence that allows the high quality images to be generated without breath-hold and without administration of contrast material. It has a potential wide application in the evaluation of hepatic vasculature and biliary tract in the follow-up of infant liver transplantation patients.
