      目的 探讨胃癌螺旋CT (SCT)征象与MVD及VEGF、TGF-β1蛋白表达间的关系。方法 对40例进展期胃癌病人行SCT平扫及三期动态增强扫描。40例均于检查后一周内施行全胃或部分胃切除手术、剖腹探查术。术后标本行常规石蜡包埋及5 μm切片。然后行VEGF、MVD、TGF-β1免疫组化染色。结果 ①在淋巴结转移阳、阴性组,VEGF的阳性表达率分别为85.71%(12/14)和15.38%(4/26);在肝转移阳、阴性组,VEGF阳性表达率分别为83.33%(5/6)和32.35%(11/34)。②在浆膜侵犯有、无组,TGF-β1阳性表达率分别为44.44%(12/27)和7.69%(1/13);在腹腔转移阳、阴性组,TGF-β1阳性表达率分别为80.00%(4/5)和22.86%(8/35)。③MVD值变化范围为4.8~49,平均值26.04(标准差11.96)。在VEGF阳、阴性组,MVD值分别为30.91±12.22和22.78±10.83;在TGF-β1阳、阴性组,MVD值分别为29.80±12.43和24.42±11.60。④在40例胃癌中,共有9例胃癌出现VEGF与TGF-β1阳性表达(P=0.002,r=0.468)。结论 VEGF的表达与胃癌SCT征象上的淋巴结转移及肝转移有关;TGF-β1的表达与胃癌SCT征象上的浆膜侵犯及腹腔转移有关; TGF-β1可能通过上调VEGF的表达而间接参与胃癌的血管生成。

      Objective To evaluated the correlation between SCT features, pathology, MVD, expressions of VEGF, TGF-β1 in gastric carcinoma. Methods Both plain SCT scan and triphasic dynamic contrast-enhanced SCT scan were performed in 40 patients with advanced gastric carcinoma. All 40 patients underwent total, subtotal gastrectomy and exploration laparotomy within a week after SCT scanning. After operation, the specimens were embedded with paraffin routinely. Five-micrometer-thick sections were obtained from the paraffin-embedded specimens. Detection procedure for VEGF, MVD, TGF-β1 were performed using immunohistochemistry. Results ①The VEGF-positive rates were 85.71% (12/14) and 15.38% (4/26) in patients with and without lymph node metastases respectively. The VEGF-positive rates were 83.33% (5/6) and 32.35% (11/34) in patients with and without liver metastases respectively. ②The TGF-β1-positive rates were 44.44% (12/27), 7.69% (1/13) in patients with and without serosal invasion respectively. The TGF-β1-positive rates were 80.00% (4/5) and 22.86% (8/35) in patients with and without peritoneal metastases respectively. ③Microvessel densities ranged from 4.8 to 49 with a mean of 26.04 (standard deviation, 11.96). The microvessel densities were 30.91±12.22 in VEGF positive tumors and 22.78± 10.83 in VEGF negative tumors.The microvessel densities were 29.80±12.43 in TGF-β1 positive tumors and 24.42±11.60 in TGF-β1 negtive tumors. ④ In 40 patients, both VEGF expression and TGF-β1 expression were detected in 9 patients, P=0.002, r=0.468. Conclusion The VEGF expression was closely correlated with lymph node metastases and liver metastases of SCT features of gastric carcinoma. The TGF-β1 expression was closely correlated with serosal invasion and peritoneal metastases of SCT features of gastric carcinoma. TGF-β1 may be indirectly correlated with angiogenesis through an up-regulation of VEGF expression in gastric carcinoma.
